In reviewing Sites 33 and 35 of the permit drawings and Site 17 of the buffer drawings, a couple of issues
were found with reporting of the impacts. All the impacts were depicted correctly on the design plan sheets
but not necessarily on the summary sheets except for Site 33. All impact changes center around Stream
SAA and SAB. Both are unnamed tributaries to Rocky Branch (SAD). A revised permit application
summary and revised permit application Tables 2, 6, 10 ,11 and 23 have been included with this memo
along with Permit Drawing Sheets 65-68, 76-77, 106, 113, and 115-119 and Buffer Drawing Sheets 11 and
19-22.
The following paragraphs summarize the impact change at each site/location.
SAB: Site 35 at station 50+01-YS to 15+32 YSA
Permanent and temporary stream impacts for SAB were missed in the original application. Stream SAB
(UT to Rocky Branch) impacts were correctly shown on Pernut Drawing Sheets 106 and 113. They were
not shown on Permit Drawing Impact Summary Sheet 117. The impact total for this station is 941 linear
feet of permanent stream and 201inear feet of temporary stream impact. Permit Drawing Impact Summary
Sheet 117 has been revised.
SAB: Site 35 at station 47+58 to 49+64-YS-RT
This station is a combination of SAB and SAA (Pernut Drawing Sheets 65-68). At this station, there were
94 linear feet of permanent impacts for SAB and 210 linear feet of permanent stream impacts for SAA,
totaling 3041inear feet. There are 351inear feet of temporary impacts for SAA and O linear feet for SAB.
This information was correctly presented on Permit Drawing Impact Summary Sheet 116. The information
was not correctly presented on Table 10. Table 10 in the permit application separated the stream impacts
per stream segment. Table 10 was revised to include the break out of the above-mentioned impacts. For
SAB this equals 941 + 94 =1, 0351inear feet of permanent stream impacts and 201inear feet of temporary
impacts. SAA impacts far Site35 will be discussed further in the section entitled: SAA: Site 35 at station
196+35 to 200+02-L-LT.
Bank stabilization is a different matter. In Table 10, bank stabilization was 79 linear feet for SAA. Part
of this stabilization belongs to SAB. SAB was not previously listed on Table 10. SAB has 441inear feet of
bank stabilization. Therefore, bank stabilization for SAA has been reduced to 351inear feet (791f — 441 fl.
Bank stabilization has been modified to reflect this change on Table 10. Bank stabilization on Permit
Drawing Impact Summary Sheets 116 and 117 were correct.
SAA: Site 35 at station 196+35 to 200+02-L-LT
SAA impacts have been adjusted to include 341inear feet of permanent impacts at this station. The stream
impact was depicted correctly on the Pernut Drawings 65 and 76-77. This stream impact was not included
as part of the impact on Permit Drawing Impact Summary Sheet 116. The additiona1341inear feet is located
beneath a proposed rip rap channel (shown on Permit Drawing Sheet 68). Permit Drawing Impact Stuliinary
Sheet 116 has been revised to 443 linear feet (409 lf + 34 1�. Table 10 has been revised to reflect this
change. The total stream impacts for SAA for Site 35 has been revised from 779 to 719 linear feet. The
amount of 719 was calculated by subtracting 941inear feet from station 47+58 to 49+64-YS-RT for SAB
and adding 341inear feet from this station. Temporary impacts for SAA remain unchanged.
SAA: Site 33 at station 47+12 to 48+74-YS-LT
SAA impacts have been updated to show the impact at the confluence of SAA and SAB at Site 33. This
impact was missed in the original permit application. The additional impact for the confluence area is 27
linear feet. This impact was not originally depicted on Permit Drawing Sheet 65 and was not shown on
Table 10. The new total is 2781inear feet (271f + 251 lf (original impact)). Permit Drawing Sheet 65 and
Table 10 for Site 33 have been updated. Permit Drawing Impact Summary Sheet 116 has also been updated.
Site 17 — Buffers at Station 202+41-L-
During the review of stream impacts for SAB and SAA the buffer impacts were re-checked to determine if
buffer impacts were accurate. A discrepancy was noted at Site 17 near the confluence of SAA and Rocky
Branch (SAD) on Buffer Drawing Sheet 11. The buffer impacts were shown correctly on Buffer Drawing
Sheet 11, however, the buffer impact amount for SAA was not correct on Buffer Permit Drawing Impact
Summary Sheet 19 and Table 11 of the permit application. Buffer impacts in Zone 1 have increased by
1,187 square feet and impacts in Zone 2 have decreased by -3 square feet. After the review, the amount of
impacts that decreased from Zone 2 at Site 17 have been shifted to Zone 1 buffers. Table 11 has been revised
to reflect this change. The Buffer Permit Drawing Suinmary Sheet 19 has been revised. Zone 1 is 119,892
(118,705 (original) + 1,187) square feet and Zone 2 is 67,192 (67,195 (original) - 3) square feet at Site 17.
Also, Site 17 has been divided further to break out SAA from SAB buffer impacts. SAA and SAB impacts
were previously combined on the original Buffer Permit Drawing Summary Sheet. The totals given above
are SAA and SAB impacts combined for Zone 1 and Zone 2.
Miti a�
Due to the revisions made for SAA and SAB stream impacts and buffer impacts for SAA, additional
mitigation will be required from NC Division of Mitigation Services (NCDMS). An additiona161 linear
feet of stream,1,187 square feet of Zone 1 buffers, and minus 3 feet of Zone 2 buffers for stream SAA will
be needed. An additional 941 linear feet of stream for SAB will be needed. The total stream mitigation
needed for this revision is 1,002 linear feet. A NCDMS request letter has been sent and the acceptance
letter will be forwarded upon receipt.

Neuse 03020201: Stream = 10,260 (941+61+9,258), Wetland = 15.23 ac, Riparian, Buffers = 562,757
(561,570 +1,187) sq. ft for Zone 1 and 367,633 (367,666 -3) sq ft for Zone 2
Cape Fear 03030004: Stream = 102 ft, Wetland = OA 1 ac, Non-Riparian Buffers = 0.01

For R-2721A, proposed impacts to jurisdictional areas total 15.24 acres of permanent wetland (riparian and
non-riparian) impacts, 11,430 linear feet of permanent stream impacts (including 1,068 lf of bank
stabilization), 461 linear feet of temporary stream impacts, 1.93 acres of permanent surface water impacts,
and 1,167,623 square feet of Neuse riparian buffer impacts. Additionally, R-2721A has 0.02 acres of
isolated wetland impacts and 0.21 acres of isolated pond impacts.
