GENERAL PERMIT INFORMATION:
• No permits lapsed or expired prior to renewal
SIU INFORMATION:
Mars Petcare (IUP # 0011, Pet Foods)
SNC and other INFORMATION:
SIU was not in SNC this PAR Year.
Please note on the IDSF form that: Arsenic, Cadmium, Chromium, Lead, Mercury, Nickel, Selenium,
Silver, Molybdenum and Ammonia were listed as not required for the 2°d 6 month period because our
LTMP annual monitoring requirements for these parameters were met in the 1" 6 month period.
Cyanide is not required on our LTMP.
