subject application. Additional information is required before we can continue our review.
Please address the following by February 15, 2001:

building beside the settling basin. Please clarify the source of the flow for this drain.

further requests may be necessary to address these comments. Please be aware` -that you are
responsible for meeting; all requirements set forth in North Carolina rules and regulations. Any
oversights that occurred in the review of the subject application package are still the
responsibility of the applicant. In addition, any omissions made in responding to the above items

01010 SUMMARY OF WORK
Part I. General
1.1 Scope of Work
This specification covers the work necessary to install a washout water recovery system for the
Caldwell Ready Mix, Inc. concrete batch mixing plant. The existing sedimentation/seepage basin
will be removed from operation.
1.2 System Description
This system is designed to treat wastewater generated from cleaning concrete batch truck mixing
drums and from washing the exterior of the batch trucks. It also receives stormwater from the
process area.
Return unused mix is discharged directly to an 8 ft DIA slurry tank. The drum interior is deluged
and the contents are also expelled directly into the reclaim system. The sand and gravel in the
return mix and washout water settle and move out from the center of the slurry tank propelled by
the cyclonic flow induced by the tank agitator. At the perimeter, the sand and gravel are swept
into a screw conveyor and carried up and out of the tank. The sand and gravel are washed in the
conveyor to remove fines and discharged onto a screen deck where they are mechanical screened
into individual stockpiles. The slurry is transferred to the 12 ft DIA agitation tank where the
remaining solids are held in suspension. The slurry is periodically returned to the batch mix
process for recycle to the product.
The wastewater originating from cleaning the batch truck exterior discharges onto a concrete slab
sloped to a shallow settling basin. Most of the solids settle in the basin, which is sloped to allow
removal of the sediment with a front-end loader. The water from the basin is pumped back to the
batch mixer for process use. A portion of the reclaimed water is used in the batch truck makeup
tank for use in adjusting the mix off site.
All stormwater from the truck washing and the process operations areas will be directed to the
settling basin. During 'storm events of greater than approximately 0.50 inches the basin will
overflow to an outfall on the west side of the plant. Stormwater from two other areas are not
routed through the recycle system. Flow from the parking area and run-on from offsite will be
directed to the east outfall. The roof drainage from the garage will be directed to a third outfall.
1.3 Project Description
The project shall consist of backfilling the existing sedimentation lagoon and removing the stock
pile, site grading, construction of slabs and the new sedimentation basin, installation of mechanical
equipment, and site drainage grading for diversion of surface run-off.

1.4 Regulatory Requirements
All work, materials, supplies, and equipment shall be in compliance with local, state and applicable
federal codes and regulations. Final installation shall not commence until all necessary permits
and approvals have been obtained.
1.5 Erosion Control
An Erosion/Sedimentation Control Plan will not be required for the project. However, prior to
construction, proper protection of the nearby stream shall be obtained by installing protective
measures to prevent excessive siltation from impacting the stream. These protection measures
will be maintained throughout the construction process. Any temporary sedimentation basins
constructed during the construction phase shall be maintained in a manner to prevent sediment
from flowing into the stream. Protection features will be removed at the completion of
construction as necessary by removal and disposal of siltatious materials. All denuded areas shall
be properly grassed and mulched as soon as practicable.
Part II. Products
2.1 Concrete
All concrete shall have a minimum of 3,000 psi compressive strength and a slump of 5 inches or
less.
2.2 Equipment
All equipment, pumps, conveyors, hoppers, etc. shall be consistent with the requirements of the
project. The mechanical equipment shall be as manufactured by Enviro-Port, Inc., Gratiot,
Wisconsin or equal. Alll mechanical equipment including pumps, shall be provided by the Owner
and installed by the contractors.
2.3 Reinforcing Steel
All reinforcing steel shall be intermediate grade billet steel, deformed. bar stock, clean and free of
rust and corrosion.
2.4 Backfill Materials
Exterior walls of the recycle facility shall be backfilled with clean, well compacted soil and/or with
flowable backfill material' as manufactured by the on site concrete batch plant.

2.5 Piping
All wastewater piping will be Scheduled 40 PVC pipe and made from Type 1 Polyvinyl Chloride.
Service temperatures do not exceed 140 degrees F. Pipe shall conform to the requirements of
ASTM D-1785. All socket type connections will be joined with primer and solvent cement
complying to ASTM D-2564.
2.6 Valves & Fittings
All wastewater piping valves will be true union ball valves made from PVC Type 1 Grade 1
molding compound. All fittings shall conform to ASTM D-2467 for socket type and ASTM D-
2464 for threaded type.
Part III. Execution
3.1 Definitions

Contractor - Parties providing equipment, materials, supplies and/or installation of the same.
Vendor - Any firm and/or party supplying materials or equipment only.
3.2 Construction Management
All work will be performed under contracts, purchase orders, and/or agreements administered by
the Owner. Construction, installation of equipment and start-up procedures shall be supervised
and inspected by the Engineer and any other regulatory authority as required.
3.3 Site Work
The existing sedimentation basin and, concrete spoil pile will be removed to allow installation of
the new system. The existing sedimentation basin will be filled to design grade.
3.2 Construction Management
All work will be performed under contracts, purchase orders, and/or agreements administered by
the Owner. Construction, installation of equipment and start-up procedures shall be supervised
and inspected by the Engineer and any other regulatory authority as required.

3.3 Site Work
The existing sedimentation basin and concrete spoil pile will be removed to allow installation of
the new system. The existing sedimentation basin will be filled to design grade.
3.4 Footings
All footings shall be poured in level trenches cut into natural soil or flowable backfill. Footings
shall contain #4 bars @ 8"o.c. continuous for length of footing trench.
3.5 Slabs
All slabs shall be poured on soil compacted to 95% standard proctor or cured flowable backfill.
Slabs shall be reinforced with #4 bars @ 12"o.c. both ways.
3.6 Walls
All containment walls shall be poured in concrete, 12" thick to the height indicated on plans. All
walls shall have #4 reinforcing bars @ 12"o.c. each way. Wall pours shall be continuous until
forms are filled with nolcold joints. All walls shall be keyed to the floor slabs and to the footings.
3.7 Mechanical Equipment
The mechanical system will be shipped assembled. Installation will include locating and setting
the system, providing water supply and connection to return flow recycle water, and electrical
wiring. The system shall be installed in conformance with the manufacturer's instructions.
3.8 Piping
3.8.1 Aboveground Piping
Piping shall be installed in a neat manner with lines straight and parallel or at right angles and as
shown on the drawings. Run piping so as to avoid interference with other lines and systems. All
work shall be accomplished using the best methods and procedures of recognized pipe fabrication
in a workmanlike manner in accordance with the latest revision of applicable ANSI Standards,
ASME Codes and PPI Standards.
Pipe shall be cut square, not upset, undersized, or out of round. Ends shall be carefully reamed
and cleaned before being installed. Use fittings for all changes in direction.
Pipe supports shall be selected as required to support the weight of the pipe, fittings, valves,
weight of medium transported and weight of insulation covering used. Solvent welded fittings
adequate to withstand 50 psig shall be used at all times for PVC pipe.

3.8.2 Underground Piping
Underground piping shall be laid at a minimum of 18 inches below the ground surface. The
trench shall be excavated to provide a firm continuous and a minimum of 4 inches of fine grained
sand shall be placed to cushion the trench bottom. The pipe shall be covered by sand a minimum
of 2 inches before placing additional backfill. Pressure testing of the underground piping shall be
completed after backfilling and prior to any concrete being placed. Installation of the
underground piping shall also follow the piping manufacturers recommendations.
3.8.3 Encapsulated Piping
Aboveground piping installed within the concrete forms and covered by the concrete shall be
installed as in Section 3.8.1 with supports adequate to hold the piping in place during the pour.
The piping shall be located so that the structural strength of the concrete walls is not significantly
impaired.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 4,000 GPD wastewater recycle system -consisting of a truck wash out pad,
a hopper for separating aggregates, mechanical screw screening conveyors, stockpile slabs, primary
separation slab, three (3) clarification basins ( approximately 9,090 gallons, 8,525 gallons and 22,740
gallons), and all necessary pumps, valves and appurtenances to serve Caldwell Ready Mix, Incorporated's
truck washing and concrete operation with no discharge of wastes to the surface waters, pursuant to the
application received October 26, 1992 and in conformity with the project plan, specifications, and other
supporting data subsequently filed and approved by the Department of Environment, Health and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until November 30, 1998, and shall be
subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans, specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in
the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the. Permittee, a formal permit request must be
submitted to the Division of Environmental Management accompanied by an application
fee, documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of the request -will be considered on its merits and may or may
not be approved.
5. Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
6. In the event Arai Lhe facilities fail to perform satisfy_ !'. ly, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as
may be required by this Division, such as the construction of additional or replacement
wastewater treatment or disposal facilities.
7. The Asheville Regional Office, telephone number (704) 251-6208, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in-place
inspection can be made. Such notification to the regional supervisor shall be made during
the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
8. The sludge generated from these treatment facilities must be disposed of in accordance with
General Statute 143-215.1 and in a manner approved by the North Carolina Division of
Environmental Management.
9. The issuance of this permit shall not relieve the Permittee of the responsibility for damages
to surface or groundwaters resulting from the operation of this facility.
10. The facilities shall be effectively maintained and operated as a non -discharge system to
prevent the discharge of any wastewater resulting from the operation of this facility.
11. Diversion or bypassing of untreated wastewater from the treatment facilities is prohibited.
12. Any monitoring deemed necessary by the Division of Environmental Management to insure
surface and ground water protection will be established and an acceptable sampling
reporting schedule shall be followed.
13. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to
insure proper operation of the subject facilities.
14. The Permittee or his designee shall inspect the wastewater recycle facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to
the release of wastes to the environment, a threat to human health, or a nuisance. The
Permittee shall keep an inspection log or summary including at least the date and time of
inspection, observations made, and any maintenance, repairs, or corrective actions taken
by the Permittee. This log of inspections shall be maintained by the Permittee for a period
of three years from the date of the inspection and shall be made available upon request to
the Division. of Environmental Management or other permitting authority.
15. Any duly authorized officer, employee, or representative of the Division of Environmental
Management may, upon presentation of credentials, enter and inspect any property,
premises or place on or related to the recycle system at any reasonable time for the purpose
of determining compliance with this permit; may inspect or copy any records that must be
kept under the terms and conditions of this permit; or may obtain samples of groundwater,
surface water, or leachate.
16. The annual administering and compliance fee must be paid by the Permittee within thirty
(30) days after being billed by the Division. Failure to pay the fee accordingly may cause
the Division to initiate action to revoke this permit as specified by.15 NCAC 2H .0205
(c)(4)
17. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
19. The issuance of this permit does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be imposed by other goverment
agencies (local, state, and federal) which have jurisdiction.

18. A set of approplans and specifications for the subj&roject must be retained by the
Permittee for the life of this project.
19. Noncompliance Notification:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number
(704) 251-6208, as soon as possible, but in no case more than 24 hours or on the next
working day following the occurrence or first knowledge of the occurrence of any of the
following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of
significant amounts of wastes which are abnormal in quantity or characteristic, such
as the dumping of the contents of a basin or tank; the known passage of a slug of
hazardous substance through the facility; or any other unusual circumstances.
b. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater treatment such as mechanical or electrical failures
of pumps, aerators, compressors, etc.
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-
pass directly to receiving waters without treatment of all or any portion of the
influent to such station or facility.
d. Any time that self-monitoring information indicates that the facility is not in
compliance with its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter
form within 15 days following first knowledge of the occurrence. This report must outline
the actions taken or proposed to be taken to ensure that the problem does not recur.
20. Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of
the wastewater treatment facilities. The operator must hold a certificate of the type and
grade at least equivalent to or greater than the classification assigned to the wastewater
treatment facilities by the Certification Commission. The Permittee must also employ a
certified back-up operator of the appropriate type and grade to comply with the conditions
of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at
least weekly and each Class II, III, and IV facility at least daily, excluding weekends and
holidays, and must properly manage and document daily operation and maintenance of the
facility and must comply with all other conditions of Title 15A, Chapter 8A, .0202. Once
the facility is classified, the Permittee must submit a letter to the Certification Commission
which designates the operator in responsible charge within thirty days after the wastewater
treatment facilities are 50% complete.

request at least 180 days prior to the permit's expiration date, as required by the 15 NCAC 2H .0211.

On March 14, 1996, the Division of Water Quality received your Notice of Intent for
NPDES Stormwater General Permit coverage under NCG140000 for stormwater discharges from
the subject facility.
In discussion with Bill Mills of the Stormwater Group concerning the Notice of Intent, you
advised that stormwater runoff from your facility discharges to a large basin at this facility which
operates under Permit No. WQ0007169 as a non -discharging system. Our Asheville Regional
Office has recently visited your plant and has advised that due to the non -discharge system which
receives your stormwater, this facility does not have a direct discharge to the waters of the state and
does not need 'a NPDES permit. Based upon.these factors your Notice of Intent is being returned
to you without any further processing actions herewith. Your application fee of $400, is being
refunded to you under a separate cover.

In response to your memorandum (received October 29, 1996)
concerning a stormwater permit for subject facility, I visited
the site on January 5, 1997. Information gathered during the
inspection revealed that Caldwell Ready Mix is operating a non
discharge system for stormwater. Consequently, the facility does
not need a stormwater discharge permit.
The facility currently holds Non -Discharge _ Permit Number
WQ0007169 and appeared to be operating substantially in
compliance with the Permit's requirements.. One deviation from
the permit was noted; the facility's owner had not installed the
recycle equipment described in the permit.
The employee onsite (Mike) at the time of inspection stated that -
there had never been a discharge from the basin receiving both
stormwater and water from equipment rinsing. Observation
revealed no -evidence.of overflows from the basin.
Sand, gravel and other settled materials are reportedly dredged
from the -basin approximately twice per year. Dredged materials
are given t(o individuals for us on driveways and roadways.

into the ground within a couple of days. Such percolation seemed
possible since the area draining to the basin was small and the
dikes forming the basin were above grade on three sides.
Apparently, sufficient freeboard exists to handle the Company's
washwater during the period required for stormwater percolation.
Caldwell Ready Mix is operating in compliance with the
requirements of Non -Discharge Permit Number WQ0007169 (except for
having constructed recycle equipment). Since the permit will
expire next year, DWQ should not force the permittee to obtain a
modified non -discharge permit at this time.
Regional Staff will recommend removal of references to recycle
equipment at the time of permit renewal in 1998. Since the
facility does not discharge stormwater, a general permit for
stormwater.will not be required.
Your attention to this matter is appreciated. Please contact me

We have pending a NOI for a Stormwater permit from Caldwell
Ready Mix, in Hudson. The NOI indicates no discharge of stormwater.
I called Mr. Triplette and discussed where his stormwater drains and
whether he had a point source discharge of stormwater from the
existing operations. He explained that all of the runoff goes to the
rear of the property into a huge basin from which there is no
discharge except seepage. He also holds non -discharge permit no.
WQ0007169 that describes a system of basins and pumps for
treatment and reuse of the washwater from this plant. I asked him
about that system and he advised that they never built that system,
it all looked too complicated. He sends his washwater from the
trucks and equipment to the same large basin from which there is
apparently no discharge. About every year or two he sends a
dragline or backhoe of some type down there and cleans the sand
and stone out of the basin.
If this facility needs a discharge permit for stormwater and/or
washwater, the NCG1400000 permit can cover it. The GP has
allowance for the discharge of both stormwater and for discharge of
rinse water associated with vehicle and equipment cleaning from
these ready mix plants. If he is effective operating a non -discharge
system for stormwater that is not a problem, there is no permit
required for the stormwater systems that operate without a point
source discharge up to the 25 yr, 24 hr. storm. I would appear that
some alteration to WQ0007169 may be in order for the washwater.
Please advise concerning your recomendations as to whether a NPDES
permit is appropriate for this facility. (He already has one permit
from us he is not using, I don't want to compound things. by issuance
of the incorrect permit. Your assistance in this matter is
appreciated.
