III. Certification Statement:
1 Ed Kerwin certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the
receiving wastewater treatment facility and that the flow from this project is not anticipated to
cause any capacity related sanitary sewer overflows or overburden any downstream pump station
en route to the receiving treatment plant under normal circumstances, given the implementation of
the planned improvements identified in the planning assessment where applicable. This analysis
has been performed in accordance with local established policies and procedures using the best
available data. This certification applies to those items listed above in Sections 1 and 11 plus all
attached planning assessment addendums for which 1 am the responsible party. Signature of this

Enclosed are the following items for your review and approval. The project includes extension and
relocation of OWASA's sanitary sewer system in order to serve a new, 328 unit apartment complex. No
changes are proposed to the number and types of dwelling units, water demand or waste water allocation.
The additional length, approximately 453-ft of 8-inch gravity sewer, is now requested in order to
accommodate an upcoming, adjacent project. The previously approved Permit included 134-ft of 8-inch
gravity sewer. The new total length is 587-ft of 8-inch gravity sewer.

Hi Pete,
Thanks for contacting us. Based on the email chain below, this modification issue first came up in July 2018. At that
time, you/permittee were informed that due to the high volume of applications we receive coupled with staff's other
duties, it typically takes us around 6-8 weeks to review applications and issue permits. We didn't receive the
application requesting the modification until January 16, 2019.
With that said, we will try to issue the modified permit in the next couple of weeks (which means reviewing it before
other applications that were submitted earlier).
You may contact us in 7-10 days to see where we are on this one.

Thanks for getting back to me so quickly on this. As we discussed, the Hillstone project is currently under
construction. As indicated in my 12/6/18 letter (attached), an expedited review to modify Permit No., WQ0O39713 is
being requested to allow for the installation of an additional 330 if of 8" DIP within Novus Lane. At present, not having
this modification to install the additional sanitary sewer is delaying the project. We would greatly appreciate an
expedited review for this modification to avoid any further project delay. If you have any questions or need any
additional information, please contact me.

A fast track sewer extension application with the $480 application fee will need to be submitted for the adjacent
property project. We are 6-8 weeks out from receipt of application to issuing permits. Explain fully and clearly in the
narrative for the application why a permit is needed sooner than the standard 6-8 week turnaround. We'll do our best
to turn it around asap.

The Hillstone project which was approved back in December 2017 (attached) is presently under construction. The
adjacent property to the northeast is now being redeveloped and approximately 335 If of additional 8" sanitary sewer
main will need to be installed within the Novus Lane ROW. As we are already under construction with the roadway
improvements, what would be the most expeditious manor in which we can file for this modification. Please let me
know as soon as you have a chance, how we can file this modification and what fees (if any) would be necessary.

Thanks for getting back to me so quickly on this. As we discussed, the Hillstone project is currently under
construction. As indicated in my 12/6/18 letter (attached), an expedited review to modify Permit No., WQ0039713 is
being requested to allow for the installation of an additional 330 If of 8" DIP within Novus lane. At present, not having
this modification to install the additional sanitary sewer is delaying the project. We would greatly appreciate an
expedited review for this modification to avoid any further project delay. If you have any questions or need any
additional information, please contact me.

Peter,
A fast track sewer extension application with the $480 application fee will need to be submitted for the adjacent
property project. We are 6-8 weeks out from receipt of application to issuing permits. Explain fully and clearly in the
narrative for the application why a permit is needed sooner than the standard 6-8 week turnaround. We'll do our best
to turn it around asap.

The Hillstone project which was approved back in December 2017 (attached) is presently under construction. The
adjacent property to the northeast is now being redeveloped and approximately 335 If of additional 8" sanitary sewer
main will need to be installed within the Novus Lane ROW. As we are already under construction with the roadway
improvements, what would be the most expeditious manor in which we can file for this modification. Please let me
know as soon as you have a chance, how we can file this modification and what fees (if any) would be necessary.

sanitary sewer extension permit. This modification consists an additional +/- 330 If of 8" DIP line within
the Novus Lane ROW for a total project length of 587 If of 8" DIP sanitary sewer main. For reference,
attached is a copy of the permit which has been issued by your office. With this being a modification
