The inspection included a review of the Application Fields, Spill Plan, and Hauling Records for the trucks used during the application event. A review of the material loading area at the Fourth Creek WWTP was also performed as part of the inspection. The MRO would like to thank Erik Bruce of Synagro and Kenneth Galloway from the City of Statesville WWTP for their time during these inspections. Laboratory TCLP data was received for the residual material as part of the inspection. The data indicated no exceedances for metals pollutant loading rates for the material being applied. PAN values appeared to be reasonable. The fields were in good condition and the material appeared to be being evenly applied across the application area. The Statesville Third Creek Class "B" residuals are included as a material source for this permit. The residual cake was applied to permitted fields NC IR 20 Fields 4A and B. Signage was observed to be present at the entrance to the fields. The fields are on private property that and not accessible to the public. This application event began Saturday 2/2/2019. The residuals from the Third Creek WWTP are stored at Fourth Creek WWTP pad. The application of the residuals is expected to continue through 2/6/2019 to be able to remove as much material as possible from the storage pad area at the Fourth Creek WWTP. During the application event, it was observed that the cake material being applied was adhering to the surface. The slope of the application fields was observed to be less than a 10 percent slope. No odors or vectors were observed to be present during the site visit. The loading area at the Fourth Creek WWTP was observed during the site visit. The cake material on the pad was approximately 10 percent solids and able to flow. In an attempt to increase the percent solids of the cake residuals,a small amount of the class "A" Sta-Lime product produced under permit WQ0006245 was blended with the Class B residuals to increase the percent solids to between 12 to 13 percent so that the product no longer had the ability to flow and that transport could occur without the potential for material leaking from the application truck. A front end loader was used at the loading area to perform the blending and load the application trucks. It was estimated that the loader shovel size was approximately 1.5 cubic yards. The total volume loaded into each truck was approximately 15 cubic yards/truck. The permit was viewed during the site visit and the field features were discussed. Two trucks were used to transport material to the application field. The spill plan was present in both haul trucks along with haul records. However, separation between surface and top of soil saturation appears to meet the 1/3 bls permit requirement. The application area that was observed during the site visit indicated that the residuals were staving in place and the residuals had been evenly distributed. No drainage features were present on the applications fields. The rain gauge at the application field was observed to be dry. Page: 4
