The State requires the Division of Water Resources to inspect these types of systems every five years. Miss Park was able to locate the effluent discharge pipe. There was no odor or noticeable impacts to the stream. Effluent being discharged in the stream should be checked periodically to ensure that no solids are being discharged and that there are no impacts to the stream. The effluent pipe is located near the back of the house, about five feet away from the stream. There were no noticeable impacts to the lawn where the outflow would flow towards the stream. Ultraviolet lights are used to disinfect the wastewater before reaching the stream. Residual chlorine monitoring is not required for this system. The lights are maintained during the yearly inspection you have done to the system through AQWA Inc. While the lights are checked during the inspection, it is strongly recommended that the lights be periodically checked throughout the year to ensure that they are functioning properly and continuing to disinfect the wastewater prior to discharge. The permit also requires that the septic tank contents be pumped out every 3-5 years to prevent solids from clogging the sand filter system. Mr. Holder emailed AQWA Inc. and it was reported that the system was pumped sometime between March and September 2015. During the last inspection done by AQWA Inc. in August 2018, the cake layer was minimal. This resulted in it not being pumped this year. Please remember to keep ' documentation for your records each time the system is pumped. NORTH CAROLINA DepmCnenl of Hnhmmendl GwllIy North Carolina Department of Environmental Quality I Division of Water Resources Winston-Salem Regional Office 1450 Hanes Mill Road,Suite 300 I Winston-Salem,North Carolina 27103 336.776.9800 108 Brook Ln.,October 22,2018 AQWA Inc. analyzes the effluent during their inspection. They analyze the effluent sample for nitrogen, biological oxygen demand, fecal coliform, and ammonia (ammonia is not a permit requirement). It is a permit requirement to analyze for the total suspended solids as well. Please have AQWA Inc. add this next time the system is inspected. Also, the estimated yearly flow out of the wastewater system should be reported. Failure to properly operate and maintain the system can result in expensive repair costs as well as failure of the system to properly treat the wastewater. If you have any questions regarding maintenance, operations, the permit, or this inspection do not hesitate to call us. Please keep this letter with your records for the system along with copies of invoices from the Division for the annual permit fee, the Certificate of Coverage, records of septic tank pumping, maintenance and repair records, etc. - Please be aware that violations of your NPDES permit, or the NC statutes and regulations under which it is promulgated, are subject to fines of up to $25,000 per day, per violation, as set forth in NC General Statute (NCGS) 143-215.6A, Enforcement Procedures, Civil Penalties. Please contact Miss Park at (336)-776-9689 or kelli.park@ncdenr.gov with any further questions. Sincerely, ..iltiolpt...4_, (3/1....9.6 j_____ 4 Sherri V. Knight, P.E.
