I sent it via USPS, and I just realized I have it addressed to Rick Bolich at 3800 Barrett Drive, rather than you at the Mail Service Drive mailing address. Do you think that will be an issue? As for "Additional Information", I will explain my understanding of how I thought the attachments met the requirements, but please let me know if something more explicit is necessary: a. The entire property boundaries of the Site where the well will be installed is visible on View 3 and View 4 (PDF pages 10 and 11) , and the adjoining roads are labeled (Shannon Road and University Drive). Do I need to have measurements of how far the property boundary is from the roads? b. Known existing monitoring wells are shown on PDF page 7 (Figure 2, Groundwater Quality Summary Figure). I am not aware of any additional wells within 500 feet of the proposed well. c. Approximate location (to be modified if necessary based on utilities) of proposed well MW-9 is illustrated on PDF pages 7 and 9-11 d. Known historic test borings within 500 feet are the same as the monitoring wells on PDF page 7. e. Known sources of contamination within 500 feet = DSCA site DC320023 Regency (PDF page 7 to 11), and potential source of GW contamination is the Sams Club Gas Station and Former H&S Cleaners (DC320022) shown on PDF page 11.
