I made all the changes to our BIMS database yesterday, and the contact summary reports should now reflect them with
the data refresh overnight. Please let me know if everything is accurate for each of these permits. Signatory Authority is
not listed in those reports, so I’ve included a screen shot of those below. (For some reason the billing contact doesn’t
show up on the application version of NCS000171, but it is listed in the report when you run it).

I’m attaching copies of our “Permit Owner Affiliation Designation Forms” for the two Kennametal facilities I have
permitting responsibility for; Henderson, NCS000171 and Roanoke Rapids NCG030151. I’m also attaching a letter listing
