On November 29, 2018, the Division of Water Resources (Division) received your fee and application dated October 25,
2018, requesting a 401 Water Quality Certification from the Division for the above referenced project. The Division has
reviewed the information and has determined the application is incomplete and cannot be processed. The application
will be placed on hold until the following items are addressed:
1. Please provide more details regarding avoidance and minimization for this project. Please explain why the
project purpose cannot be accomplished by installing the proposed waterline along the toe of the slope of the
western side of North Fork Left Fork Road to better minimize the impacts to streams and wetlands. [15A NCAC
02H .0506(f) and (g)].
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received within 30 days, the Division
will be unable to approve the application and it will be returned.
