1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
Steps were taken to avoid and minimize stream and wetland impacts during the final design of Stage 1 B-Phase II. The original interchange concept
included ramps in all four quadrants of the Morrisville Parkway interchange with NC 540. The final design includes ramps and loops in the northwest
and southeast quadrants of the interchange. This design eliminates impacts to WC-6 and SC-8 in the southwest quadrant of the interchange without
adding any new or additional impacts. In addition, careful design of proposed grades and the use of 2:1 fill slopes minimized impacts at the remaining
sites.
1b. Specificallydescribe measures taken to avoid or minimize the proposed impacts through construction techniques:
The Contractor will be instructed to followthe erosion and sediment control plans, induding routine maintenance to devices. Specific attention will be
paid to work in the streams and wetlands as well as the management of base flows.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
C' Yes r No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:

Wetland impacts are belowthe PCN threshold per the Nationwide Permit Summary Chart (1/5/17)
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
 Yes r No
What type of SCM are you providing?
r Level Spreader
r Vegetated Conveyance (lower SHWT)
 Wetland Swale (higher SM/VT)
Other SCM that removes minimum 30 % nitrogen
J Proposed project will not create concentrated stormwater flow through the buffer

Comments:
This a linear transportation project providing storrrrvvater treatment to the maximum eMent practicable. The project will install 12 bioretention cells to treat stormwater runoff. Storrrrvvater
calculations are included with this submittal.
In regards to the diffuse flow plan, the project is not creating any new outfall locations.
