Pretreatment Annual Report 21317
Narrative
Cape Fear Public Utility Authority (CFPUA) treats industrial wastewater from six significant industrial users (SIUs) including two
categorical users. Wastewater from Alche_mi Incorporated iAlchemi). Corning Incoroorated {Corning), and UniFirst Co[eoration
LUniFirst) is discharged to the James A. Loughlin WWTP (Northside) through connections to the collection system. Wastewater from
Waste Management of Wilmington (WM) and SR&R Environmental Inc. (SR&R) is hauled to Northside. Wastewater from New
Hanover Regional Medical Center (NHRMC) is discharged to the M'Kean Maf Itt WWTP (Southside) through connections to the
collection system.
It should be noted:
• May 2016, HMF Express capped their discharge line and began shipping their wastewater out of the CFPUA service area.
The last discharge to the CFPUA sewer was April 29, 2016. No wastewater was discharged during 2017. On June 27,
2017, HMF Express requested that their permit be rescinded. CFPUA rescinded the permit effective June 29, 2017.
• June 12, 1017, the permit was modified at the request of SR&R Environmental to allow them to receive leachate
from Duke Energy Progress Sutton Plant's lined coal ash landfill.
There were no SIUs in significant non-compliance (SNC) for the reporting period. Notices -of -Violation (NOVs) and civil penalties were
assessed during the reporting period per CFPUA's Enforcement Response Plan (ERP) and are summarized below:
• Waste Management was issued a NOV for failure to sample Oil & Grease by the proper test method per sampling
frequency and failure to analyze for Oil & Grease for April 2017. They were assessed 2 points.
• SR&R Environmental:
o Was Issued a NOV for violating the daily maximum limit for titanium on July 19, 2017. The violation was TRC
and they were assessed 3 paints per the ERP. They were also assessed 3 points for failure to notify CFPUA
within 24 hours of becoming aware of the violation.
o Was issued a NOV for violating the monthly average limit for p -Cresol for October 2017. The violation was TRC
and they were assessed 3 points per the ERP.
o Was issued a NOV for violating the daily maximum limit for antimony on November 21, 2017. The violation
was TRC and they were assessed 3 points per the ERP.
o Was issued a NOV for violating the dally maximum limit for titanium on December 4, 2017. The violation was
TRC and they were assessed 3 points per the ERP.
o On February 2, 2018, was assessed a civil penalty of $5,312.50 for violations occurring during the July -
December 2017 reporting period.
• UniFirst:
o Was issued a NOV for violating the dally maximum limit for BOD on April 4, 2017. They were assessed 2 points
per the ERP.
o Was issued a NOV for violating the daily maximum limit for BOD on April 5, 2017. They were assessed 2 points
per the ERP.
o Was issued a NOV for violating the daily maximum limit for BOD on June 15, 2017. They were assessed 2
points per the ERP.
o On July 28, 2017, was assessed a civil penalty of $352.70 for violations occurring during the January -June 2017
reporting period.
• NHRMC:
o Was issued a NOV for violating the daily maximum limit for iron on July 14, 2017. They were assessed 2 points
per the ERP.
o Was issued a NOV for violating the daily maximum limit for ammonia on July 14, 2017. They were assessed 2
points per the ERP.
o Was issued a NOV for violating the daily maximum limit for ammonia on August 18, 2017. The violation was
TRC and they were assessed 3 points per the ERP.
o On February 2, 2018, was assessed a civil penalty of $220.00 for violations occurring during the July -December
2017 reporting period.

All other SIUs were in compliance for the reporting period. One IU permittee, New Anthem Brewery, was assessed a $275.00 civil
penalty for a pH violation on December 29, 2017 and for failure to notify CFPUA within 24 hours of becoming aware of the violation.
There is no change in the historical SNC status of SIUs. The following items are attached for review:

Hey Deborah,
We were supposed to have a conference call with the complex permitting group tomorrow to discuss the dilution study
and our next permit, but due to the GenX issue that is being postponed until mid July. My intention was to have that call
and then call you to discuss our options on the HWA with the new information. Our extension was for 30 days which
will be up prior to the meeting. Should I be requesting a longer extension or do you want to discuss this some more
tomorrow?
