General Program Information The status of pretreatment program documents is described below: • Short-term monitoring plan (STMP): The STMP was last updated in February 2012. The STMP was conducted in 2017. • Headworks analysis (HWA): The last HWA was approved by the PERCS Unit on February 20, 2014. The next HWA is due April 1, 2o18. • Allocation table: The allocation table was submitted on February 11, 2014. The next allocation table revision will occur with the April 1, 2oi8 HWA submittal. • Sewer Use Ordinance (SUO): The SUO was revised in 2013. The SUO will not be revised again unless updates are required by the Division of Water Resources as a result of Federal or State rule changes. • Industrial Waste Survey (IWS): An IWS was completed bythe sole industrial user in 2011. No change in dischargers occurred in 2017. B. General Permit Information Industrial User Pretreatment Permit (IUP) oo1 was renewed on August 1, 2015, incorporating a name change and reduced permitted flow rate. The IUP was renewed on JIM 23, 2017. II. SIU Information Hexion, Inc. (formerly known as Momentive Specialty Chemicals, Inc.) is permitted as the sole industrial user of the POTW. A. SIUs in Significant Noncompliance (SNC) No SIUs were in SNC during the period of January 1, 2017 through December 31, 2017. B. Orders or Schedule Information No SIU was under an Administrative Order, Consent Order, or Enforced Compliance Schedule during the period of January 1, 2017 through December 31, 2017, C. Pretreatment Authorization to Construct and Construction Information No SIU requested an Authorization to Construct during the period of January 1, 2017 through December 31, 2017. D.
