the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with Iocal established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

I. There are at least three different variations of the name in the application. We plan on calling it DaVita -- Kenly, unless
you'd Me it be culled something else.

2. In Section V1. 7 there needs to be an explanation of how the 2T rules were used to come to 5,600 GPD.
Response: The flow was provided by the developer 1 architect and is based on prior projects with similar building size
and use.
3. Section VI11.5 proposed 1 %z" forcemain. The narrative states it will be 3". What is the diameter of the proposed forcemain?
Response: The proposed force main is 3" PVC. Section V11I was updated accordingly.
4. Section IX.1. was not completed.
Response: Section IX 1 wus revised to show compliance with referenced code f -setbacks.
5. The engineers name should be printed in the space provided in Section X.

We received the application on November 51". With the volume of applications we receive coupled with our other
duties, it's currently taking us around 6-8 weeks to review and issue permits.
Unfortunately it may not be until after Christmas before the permit is issued. Sorry for the delay.

We just received and responded to comments from DEQ regarding the sewer applications for this project and wanted to
send you the revised application for your review. I highlighted the revisions, in the attached / revised application, for
your convenience and our comment responses to DEQ are below:

After review of the water and sewer application we found that we need a few more things to complete the process.
Approval of Eng, Plans for Water Supply Syst. form:
* Need an additional Signature on page 1(highlighted)
o Do you have an approved standard specifications (page 4 of 4, highlighted) or any other specs we can attach
to the submittal?
• Engineer's Report:
o Page 4 — name of person who provided the data (Highlighted)
• Sewer extension fast track application
o Page 5 of 5 — Nanme and signature of applicant (Michael Douglas)

Fast track) applications which require input from DPW (as the applicant) for the water and sewer system extensions.

Utility Plan for your convenience.
