Please insert these change pages into your existing permit and discard the old pages. We regret any inconvenience this causes your organization The final permit includes the following significant changes from the existing permit: 1. A compliance period of six(6)months from the permit modification effective date has been added in Section A. (1) as footnote#9 allowing weekly monitoring for pH, dissolved oxygen, BOD, total suspended solids, and upstream and downstream temperature. At the end of this six (6)months,monitoring will revert to 3/Week as stated in the permit. 2. A compliance period of six (6)months from the permit modification effective date has been added in Section A. (1) as footnote#10 allowing 2/Week monitoring for total residual chlorine. At the end of this six (6)months,monitoring will revert to 3/Week as stated in the permit. Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain any other Federal, State, or Local governmental permit that may be required. Page 3 of 9 Permit NC0031828 Footnotes: 1. The permittee shall submit discharge monitoring reports electronically using the NC DWR's eDMR application system [see Section A. (6)]. 2. Upstream=at NCSR 1440. Downstream=approximately 2 miles downstream from the outfall. 3. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance with the permit.However,the Permittee shall continue to record and submit all values reported by a North Carolina certified laboratory(including field certified), even if these values fall below 50 µg/L. 4. The monthly average effluent BOD5 and TSS concentrations shall not exceed 15%of the respective influent value(85%removal). 5. For a given wastewater sample,TN=TKN+NO2-N+NO3-N,where TN is Total Nitrogen and TKN is Total Kjeldahl Nitrogen. 6. TN Load is the mass load of Total Nitrogen discharged in a given period of time. See Section A. (2) Calculation of Total Nitrogen Loads. 7. The monthly average for total phosphorus shall be the average of composite samples collected monthly. 8. Mercury must be monitored once during the permit cycle. See Section A. (5)Effluent Mercury Analysis. 9. Monitoring will be maintained at weekly until six(6)months from the permit modification effective date, at which time the 3/Week sampling will be implemented. 10. Monitoring for total residual chlorine will remain at 2/Week until six(6)months from the permit modification effective date, at which time the 3/Week sampling will be implemented. ALL SAMPLES SHALL BE FROM A REPRESENATIVE DISCHARGE EVENT. THERE SHALL BE NO DISCHARGE OF FLOATING SOLIDS OR VISIBLE FOAM IN OTHER THAN TRACE AMOUNTS. Page 4 of 9
