Dear Mr. Kichefski:
The Not�h Caralina I3epartment of Taransportation is scheduled ta replace Eridge #83 on
SR 1340 with a new brzdge in the sam� loeation. The existing brid�e is a 19' Iong x 18'
wide timber bridge with t�mber joists built in 1965. T�e praposed bridge is 30' la�g x
2$' wide with prestressed cancrete cored slahs. An on-site detour will be used d�ring
construction by installing twa temporary 60" CSPs just upstream of the bridge. Due to
the realignment of a nearby drivEway, a short stream reloeation of a UT in the driveway
ditch lin.e will be needed. Stream impacts are tabulated below.

Please note that water is x�.uini_ng in the ditcl� line along SR 1340 in the southern quadrant
of the bridge. This water is entering the d.itch fram a small pipe south of the projec#
limits. The arigin is an underdrain system in the back yard of an adjacent property owner.
This featwre was deerned not �urisdictional.
Construction of this praject will require impacts to waters of the United States in the New
River Basin. The existing bridge crosses the North Fork New River (Class: C; Trout,
ORV�. The project is lacated within a trout watershed. This project does not drain to
designated critical habitat for the elk toe musse�.
This project was reviewed far the presence of threatened and endangered species in the
field and I have also included a query of the NCNHP database. Based on the project axea
rnapped with my request, the database search indicates that there are na records of any
federally listed threatened ar endangered species within the project limits. The stream
relocation will hawever cIip the corner of Elk Knob State Park. Our staff will coardinate
with the State Property Office to resoive thi� issue during the right-of vcray phase. There
wili be 0.02 acres of tree ciearing at the praject laca��on due ta cut and fiil work. The
NLEB is listed as a protected species in #his county but the project does not fall within
fl.2S miles af a.ny red HCTC identified in the SLOPES spelled out in the Nationwide
Pertnit Regional. Conditions. We do not anticipate any impact to any other federally
Iisted species_ No archaeological resources or historic praperties will �ie affected by this
project (see attached).
The project impacts are not signif cant in nature and should nat alter the best usage for
this streazn. All appropriate BMPs will be �sed during construction. Attached is a
Preconstructian Notification Form, plan. ,sheets, and a marked topagraphy map.
By copy of this electronic letter, it is requested Yha� the Division of Waier Resources,
401/Wetlands Urut, issue the appropriate permit for construction of this project. It is
requested that any comments relatec� to the 401 certif catian be electronically forwarded
directly to me with a copy sent to the US Army Corps of Engineers.
By copy of this letter, it is requested that Mrs. Marla Chambers, Mountain Region
Coordinator, with the North Caralina Wilcllife Resources Cnmznission comment directly
to you concarning the 404 Nataonwide Permzt application with a copy sent to rne.
