Subject: Pre-Construction Notification for Culvert No. 080202 repair due to
damage by Hurricane Florence
State Project No. 15406.1009013 involves the replacement of the existing NCDOT
Culvert No. 080202 on NC 210 over Smith Mill Pond Run in Bladen County. Smith Mill
Pond Run lies within the Cape Feaz River basin. Its best usage classification is C; Sw and
its Stream Index number is 18-68-12-14. The existing structure experienced severe
impacts during Hurricane Florence. The existing Culvert No. 080202 is a 2@ll2"x75"
CMPA and will be replaced with a 2@12'x7' RCBC with beveled headwall.
Project construction will result in 951ineaz feet (0.02 acre) of permanent surface water
impact and 0.02 acre of temporazy surface water impact. An offsite detour will be used.
Please see the enclosed copy of the permit drawings for the above-referenced project.
Regulatory Approvals
Section 404 Permit• The NCDOT requests that these activities be authorized under the
Nationwide 3 permit.
Secfion 401 Permit: The NCDOT requests that these activifies be authorized under Water
Quality General Certification No. 4132 (Maintenance). We anticipate that written
approval from the NC Department of Water Resources will not be required.
