Good Morning Sue,
There was actually not an attachment from the City response to the applicant. However, it is our understanding that
the City was referring the local sanitary sewer permit approval or the previous attachment (preliminary impact exhibits)
submitted to the City in inquiry email, both of which confirm the proposed crossing location. I have attached copies of
the email correspondence and both pdf attachments as stand-alone documents. Note that the impact drawings
provided to the City were preliminary in nature and differ from those submitted within the PCN application due to
design changes. Additionally, it is the applicant’s understanding that the City of WS will be responsible for maintenance
activities of the proposed sewer line following installation.

Brad,
Can you provide a copy of the “attachment” referenced in the email from the City. Can you also clarify if the sewer is to
be turned over to the City once constructed? I’m unclear as to why the City staff would need to perform maintenance
(as referenced in the email) if it’s a private sewer (as referenced in the justification of why they could not secure an
easement).

This Permit and Authorization to Construct provides for the discharge of collected domestic wastewater into the City of Winston-
Salem's existing sewerage system, pursuant to the application received on 10-30-18 and in conformity with the requirements
of the North Carolina Department of Environment and Natural Resources (NCDENR).
This Permit and Authorization to Construct shall be valid for 24 months from the date of issuance or until rescinded, and shall be
subject to the following specified conditions and limitations:
1. This Permit and Authorization to Construct shall become void unless the facilities are constructed in accordance with approved
plans, specifications, and other supporting data.
2. Construction of the sewers shall be so scheduled so as not to interrupt service by the existing utilities nor result in an overflow or
bypass discharge of wastewater to the surface waters of the State.
3. This Permit and Authorization to Construct is effective only with respect to the nature and volume of wastes describe in the
application and other supporting data.
4. Use of the facilities cannot begin until final inspection approval has been received and a Contract of Conveyance executed by the
City and the applicant.
5. The facilities shall be properly maintained and operated at all times by the City of Winston-Salem upon acceptance by the City.
6. The sewage and wastewater collected by this system shall be adequately treated in the City of Winston-Salem's:
❑ Elledge Wastewater Treatment Plant prior to being discharged into the receiving stream.
® Muddy Creek Wastewater Treatment Plant prior to being discharged into the receiving stream.
7. A copy of this Permit and Authorization to Construct and a copy of the approved engineering plans must be made available at the
job site upon request by a representative of the City of Winston-Salem.

I have attached the updated Wetland Impact Map for Sedge Hollow Phase 2.
Bradley Luckey from Pilot Environmental has asked us to obtain a letter from the City of Winston Salem stating that the
storm pipe cannot be installed as an aerial crossing with no impact to the stream.
If you could handle that for us, that would be great!
