October 2018 Stormwater DMRs
To Whom It May Concern:
I have enclosed two (2) copies of the October 2018 Stormwater DMR for CommScope,
Inc. WWTP. The CommScope Certificate of Coverage No. is NCG030078.
For this month, CommScope, Inc. completed a wet sampling event of the required
seven (7) outfalls for the facility on Friday, October 26' , 2018. These include the
following outfalls: 003, 004, 005, 006, 008, 009, and 012. All seven (7) outfalls were
monitored for oil & grease, TSS, pH, lead, copper, and zinc.
For this samDlina event, the followina exceedances from the Dermit were noted:

The exceedances noted above are being addressed by CommScope personnel in the
following corrective action steps:
1) Control measures are still being implemented to reduce the metal concentrations
including covering / shielding operations as well as directing run-off away from
any critical outdoor areas.
2) Also, for the zinc (Zn) exceedances, it is noted that there is excessive runoff from
air condition drains from the roof. This has showed positive results in the past
for zinc (as collected from these roof drains and as stated for the December
2017 DMR).
3) Staff has also been completing and working on conducting more sampling (when
allowable) to further document corrective action measures.
4) Staff has also notified Mr. James Moore about past exceedances at the facility via
email on Thursday, November 1°, 2018 providing an excel spreadsheet to
highlight all exceedances up to September 2018 and to better understand the
data for the facility. Furthermore, staff has notified Mr. Moore on Friday,
November 23id, 2018 with updated data to reflect the October 2018 DMR report.
5) Furthermore, based on other past sampling events (as well as performing
monthly dry weather inspections), staff is working diligently by addressing any
issues as well as updating best management practices to address the above
noted benchmark exceedances.
Now, based on previous sampling events (and still working on the above corrective
action steps), I feel that CommScope, Inc. is working on addressing these exceedances
as needed and will continue to follow up with necessary Best Management Practices as
needed.

Monthly sampling (instead of semi-annual) must begin with the second consecutive benchmark exceedance for the same parameter at the same outfall.
2 For sampling periods with no discharge at any single outfall, you must still submit this discharge monitoring report with a checkmark here.
-The total precipitation must be recorded using data from an on -site rain gauge. Unattended sites may be eligible for a waiver of the rain gauge requirement.

S Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included In the Stormwater Pollution Prevention Plan."
