In accordance with your permit major modification request received October 17, 2018, and
subsequent additional information received January 2, 2019, we are forwarding herewith modified
Attachments A and B certified January 14, 2019, to Southern Soil Builders, Inc. for the subject residuals
management program.
Modifications to the subject Attachments are as follows:
1. Addition of 300 dry tons/year of residuals from one new source, the Belmont WWTP, (Permit
No. NC0021181) in Gaston County.
2. Addition of four transferred fields in Gaston County totaling 82 acres, those fields being NC-
GA-MH-02-01, -02, -04 and -05. These fields are being transferred from the Town of Dallas
(Permit No. WQ00020912).
The modified Attachments A and B shall be effective from the date of certification until June 30,
2019 (i.e., expiration date of the current permit), shall void Attachments A and B of Permit No. WQ0037135
issued July 3, 2014 that were previously certified on August 15, 2018 and May 11, 2018, respectively, and
shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -
referenced permit. Please replace Attachments A and B of Permit No. WQ0037135 that were certified on
August 15, 2018 and May 11, 2018 with the modified Attachments A and B.
Please note that only one map, Figure 13, for the new land application sites is attached to this letter.
It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents
into the previously issued permit.

If any parts, requirements or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.

2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2.
3. Testing of metals and nutrients as stipulated under permit Condition IV.3.
4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IVA.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.
