RE: Level Spreader Installation Certification

As the designer and installer of the level spreader at 419 Sagamore Drive at Lake Royale,
I am certifying that the construction was conducted in accordance with the approved
plans. Onsite revisions were made with guidance from the Division of Water Quality and
include the removal of a channel to the forebay and improved freeboard or storage on the
upslope side of the level spreader lip. See Figure 11 for final construction photographs.
Please let me know if you have any further concerns with this stormwater BMP.
