Any violation of these terms may ie suDject to nnes, imprisonment or civii action; or may cause me permit to be nun and void. 1) This renewal shall be attached to Permit No. 1-99, which was transferred to the permittee on 12/9/02, as well as all subsequent modifications, refinements, and renewals, and copies of all documents shall be readily available on site when a Division representative inspects the project for compliance. 2) All conditions and stipulations of the active permit remain in force under this renewal. NOTE: Permit No. 15-15, which was issued to Potters Marine c/o Matthew Potter on 1/29/15, was issued in error and intended to be a Major Modification to Permit No. 1-99. An Amended Major Modification to the active Permit No. 1-99 was issued on 2/18/15 as a correction. The N.0 Division of Water Resources and U.S. Army Corps of Engineers project numbers associated with this Permit were originally issued to Permit No. 15-15. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty (20) days of the issuing date. This permit must be accessible on-site to Department personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. All work must cease when the permit expires on December 31, 2020 In issuing this permit, the State of North Carolina agrees that your project is consistent with the North Carolina Coastal Management Program. NOTE: The U.S. Army Corps of Engineers has assigned the proposed project SAW Action Id. No. 2014- 02242. NOTE: A permit renewal application processing fee of $100 was received by DCM for this project.
