Dear Mr. Pleasant,
On January 4, 2019, Kelli Park and Paul DiMatteo of this office, met with you to perform a
Compliance Evaluation Inspection at Cobble Creek Lumber. This type of inspection consists of
two basic parts: an in-office file review and an on-site inspection of the treatment facility. The
attached EPA inspection form details the areas that were evaluated during this inspection.
Miss Park found the following violations during the inspection:
1) The Certificate of Coverage (CoC) is still in the previous company's name (Elk..Creek
Lumber, Inc). The contact information is also still under the old owner's information. This
needed to be changed before Cobble Creek Lumber began discharging boiler blowdown
effluent into the storm drain in August of 2017. Miss Park provided you the documentation
to change this information. Please get this done immediately.
2) There was no permit or CoC on site. Miss Park provided you a physical copy of the permit.
Please have these documents available for the next inspection.
3) No effluent monitoring has been done. Miss Park provided you with a list of the certified
laboratories in North Carolina that can help you with this. Please begin monitoring your
effluent immediately and continue to do so to remain compliant with the permit
requirements. What needs to be monitored can be seen in the permit provided. Please be
aware that the amount of flow from the boiler blowdown needs to be recorded as well.
Miss Park suggested that the employees record the daily flow in a log sheet.

4) It was brought to Miss Park's attention that there might be a difference in the type and
amount of additives to the effluent.than what was permitted for Elk Creek Lumber, Inc.
Please make any differences known to NCDEQ when submitting the name change form
for the CoC. Miss Park also suggested to you that the employees should keep a daily log
of what was added each day along with how much was added.
5) Cobble Creek Lumber has two boilers, but one is out of service due to a meltdown. Please
let Miss Park know when it has been repaired and is back online.
6) You mentioned to Miss Park that you want to move the boilers to a different location on
site. Please let Miss Park know your plans for this, when construction begins, and when it
is completed. Please keep in mind that you must be able to get samples from your boiler
.blowdown effluent to remain compliant with the permit.
