As requested, I am sending you a copy of the stormwater plan and application for Fieldstone Phase 1 at High Hampton. I
have not attached copies of the supporting calculations and other documentation. I will be glad to send that
information if that is helpful.
This package is being submitted with the Resort Core stormwater today. I will send you a separate email with the same
information on the Resort Core. We will be following up with additional submittals for Golf Maintenance, Back of
House, etc.

V. SUPPLEMENT AND O&M FORMS
The applicable state stormwater management permit supplement and operation and maintenance (O&M) forms
must be submitted for each BMP specified for this project. The latest versions of the forms can be downloaded

VI. SUBMITTAL REQUIREMENTS
Only complete application packages will be accepted and reviewed by the Division of Water Quality (DWQ).
A complete package includes all of the items listed below. A detailed application instruction sheet and BMP
checldists are available from http�portal.ncdenr.org web/wq/ws/su/statesw/forms docs. The complete
application package should be submitted to the appropriate DWQ Office. (The appropriate office maybe
found by locating project on the interactive online map at http:/ /portal.ncdenr.org/web/wq/ws/su/maw.)
Please indicate that the following required information have been provided by initialing in the space provided
for each item. All original documents MUST be signed and initialed in blue ink. Download the latest versions
for each submitted application package from ham://portal.ncdenr.org/web/wq/ws/su/statesw/forms docs.

6. A USGS map identifying the site Location. If the receiving stream is reported as class SA or the
receiving stream drains to class SA waters withul1/z mile of the site boundary, include the 1/z
Hole radius on the map.
7. Sealed, signed and dated calculations.

impounded structures, the banks of streams and rivers, the MHW or NHW line of tidal
waters, and any coastal wetlands landward of the MHW or NHW lines.

VII. DEED RESTRICTIONS AND PROTECTIVE COVENANTS
For all subdivisions, outparceIs, and future development, the appropriate property restrictions and protective
covenants are required to be recorded prior to the sale of any lot. If lot sizes vary significantly or the proposed
BUA allocations vary, a table listing each lot number, lot size, and the allowable built upon area must be provided
as an attachment to the completed and notarized deed reshiction form. The appropriate deed reshictions and
protective covenants forms can be downloaded from
http://portal.ncdenr.org/web/wq/ws/su/statesw/forms docs. Download the latest versions for each submittal.
In the instances where the applicant is different than the property owner, itis the responsibility of the property
owner to sign the deed restrictions and protective covenants form while the applicant is responsible for ensuring
that the deed restrictions are recorded.
By the notarized signatures) below, the permit holders) certify that the recorded property restrictions and
protective covenants for this project, if required, shall include all the items required in the permit and listed
on the forms available on the website, that the covenants will be binding on all parties and persons claiming
under them, that they will run with the land, that the required covenants cannot be changed or deleted

1. ALL PIPE SYSTEMS SHALL BE INSTALLED IN ACCORDANCE WITH ASTM D2321,
"STANDARD PRACTICE FOR UNDERGROUND INSTALLATION OF THERMOPLASTIC
PIPE FOR SEWERS AND OTHER GRAVITY FLOW APPLICATIONS", LATEST ADDITION
2. MEASURES SHOULD BE TAKEN TO PREVENT MIGRATION OF NATIVE FINES INTO
BACKFILL MATERIAL, WHEN REQUIRED.
3. FOUNDATION: WHERE THE TRENCH BOTTOM IS UNSTABLE, THE CONTRACTOR
SHALL EXCAVATE TO A DEPTH REQUIRED BY THE ENGINEER AND REPLACE WITH
SUITABLE MATERIAL AS SPECIFIED BY THE ENGINEER. AS AN ALTERNATIVE AND
AT THE DISCRETION OF THE DESIGN ENGINEER, THE TRENCH BOTTOM MAY BE
STABILIZED USING A GEOTEXTILE MATERIAL.
4. BEDDING: SUITABLE MATERIAL SHALL BE CLASS I, II OR III. THE CONTRACTOR
SHALL PROVIDE DOCUMENTATION FOR MATERIAL SPECIFICATION TO ENGINEER.
UNLESS OTHERWISE NOTED BY THE ENGINEER, MINIMUM BEDDING THICKNESS
SHALL BE 4" (100mm) FOR 4"-24" (100mm-600mm); 6" (150mm) FOR 30"-60"
(750mm-900mm).
5. INITIAL BACKFILL: SUITABLE MATERIAL SHALL BE CLASS I, II OR III IN THE PIPE
ZONE EXTENDING NOT LESS THAN 6" ABOVE CROWN OF PIPE. THE CONTRACTOR
SHALL PROVIDE DOCUMENTATION FOR MATERIAL SPECIFICATION TO ENGINEER.
MATERIAL SHALL BE INSTALLED AS REQUIRED IN ASTM D2321, LATEST EDITION.
6. MINIMUM COVER: MINIMUM COVER, H, IN NON -TRAFFIC APPLICATIONS (GRASS
OR LANDSCAPE AREAS) IS 12" FROM THE TOP OF PIPE TO GROUND SURFACE.

Please find enclosed the submittal for post construction stormwater permitting for a new subdivision
in the High Hampton Community in Jackson County, NC. This is a high density project located in a Tr+
waters, so post construction stormwater management is required.
With this letter, please find enclosed two copies of the stormwater management plans, supporting
calculations and narrative, completed permit application and signed operation and maintenance
agreement. Also, I have enclosed a CD with PDF files with all this information.
