Good morning. I have attached a scanned copy of the new Owner Affiliation form for all of our Waste
Management of Carolinas, Inc. facilities. The original form was signed by Eric Wakefield, Area Director of
Collections, and mailed to the address on the form. I am also attaching the Excel spreadsheet file in case it
might simplify updating your database. This database lists the District Manager for each facility who should be
the "Signature Authority Contact" for each site.
Again, we appreciate your assistance in this matter and apologize for letting the information become
outdated. Please let me know if you have any additional concerns or need any additional information.
