Mr. Tommy Spicer, Operator in Responsible Charge (ORC), was present for this inspection. This type of inspection consists of two basic parts: an in-office review of the facility files and self-monitoring data and an on-site inspection of the facility. While this inspection mostly reflects compliance with Permit No. NC0038709, the following concerns requires your attention and action: 1) Proper maintenance of the surface sands filters must be performed at a frequency that will prevent ponding of effluent and/or vegetative growth. Weekly raking is highly recommended to comply with Permit Condition Part II Section C. (2.) which requires the Permittee to properly operate and maintain all facilities and systems of treatment and control at all times. 2) Two broken lateral pipes were observed in the surface sand filters during this inspection, and in need of immediate repair. Bi-weekly or monthly observation of sand filter dosing, should be included as routine maintenance. Biological treatment may be adversely impacted by failure to evenly distribute effluent within the treatment filters. 3) The audible component of the effluent pump station alarm failed to operate when tested during this inspection. Immediately repair or replace to ensure proper operation of this audible/visual alarm. All audible/visual alarms should be tested monthly to ensure proper operation at all times. Weekly maintenance including raking of the sand filter shall be performed to prevent this from occunng. 2)Two broken lateral pipes were observed during this inspection resulting in uneven distribution of effluent within the treatment filter. Repair/replace immediately to avoid potential adverse impacts to biological treatment. _ 3)Audible alarm for the efflent pump station failed to perform during testing at the time of this inspection. Replace/repair immediately. Montly testing should be included as part of routine maintenance procedures to ensure proper operation of all alarms at all times.
