Dear Mr. Perry:
On July 21, 2009, the Division of Water Quality (DWQ) received your application dated July 15, 2009, to
fill or otherwise impact 1,479 square feet of Zone I Neuse River Basin riparian buffers, and 1,269 square
feet of Zone 2 Neuse River Basin riparian buffers to construct the proposed sanitary sewer connection at
the site. The DWQ has determined that your application was incomplete and/or provided inaccurate
information as discussed below. The DWQ will require additional information in order to process your
application to impact protected wetlands and/or streams on the subject property. Therefore, unless we
receive five copies of the additional information requested below, we will place this project on hold as
incomplete until we receive this additional information. If we do not receive the requested information,
your project will be formally returned as incomplete. Please provide the following information so that we
may continue to review your project.
Additional Information Requested:
1. Please provide an agent authorization letter for the application signature.
Signed authorization letter is attached.
2. Please provide a detail of the aerial sewer crossing. Note that any wastewater line that crosses a
stream shall be installed either a) with no joints connected within the footprint of a stream
channel or with in two feet of the stream banks in the case of plastic or PVC pipes or b) with
properly bedded and supported ductile iron. Otherwise 401 written approval from DWQ is
required.

In section A I b, you have requested a NW 12, but have included not permanent or temporary
stream or wetland impact on the PCN form. If only buffer impacts are proposed, please leave
section A 1 b blank in the future. If a written 401 authorization is required, the appropriate fee

This NWP number has been removed as we only have Buffer impacts.
