Ms. Georgoulias,
Thank you for your help. There have been no modifications at the site. The stream previously listed
on the permit is incorrect. It should be listed as Little Sapony Creek- Stream Index: 28-55-6-(0.3).
Please do send me confirmation when it has been changed in your system.
I will get the Ownership Change form submitted as soon as possible.

I apologize for the delayed response -- our group relocated offices, and most of us have not have
access to computers or phones since Wednesday afternoon.
Did the receiving water change because of modifications at the site, or was the one listed on the COC
just incorrect? Also, what is the new receiving stream and index number? You can find that
information on this map viewer if you need to.
I can make that change in our system and send you confirmation. I can't change the COC that's on-
line, but if there's an ownership change, we'll have to send the permittee a new one anyway.
We have an on-line form for an Ownership Change that can submitted to us for that. Right now I'm
getting all those, but I am about to train our new Admin Assistant to start handling those requests, so
please advise your client it may take some weeks to get the final paperwork.

Good Morning Ms. Georgoulias,
Happy New Year!
I am a consultant for Hirschfeld Industries Bridge in Nashville, NC and we have already requested a
copy of the certificate of coverage for their facility. Hirschfeld would like to make a change to the
receiving stream so that it will reflect recent changes made to their stormwater plan. The current
stream listed is Bear Branch and the change needs to be made to Little Sapony Creek. Is it possible
to make this change on the COC for their facility?
Also, Hirschfeld recently changed ownership and soon we will be in the process of changing the
permit names to reflect the new facility name. Would it be appropriate to make the change to the
receiving stream on the COC during that time? Please let me know what you suggest.
