Gentlemen:
Please refer to the PreDi.scharge Notification (PDN) dated October
.6, 1992 requesting approval of disturbance of 1.7 acres of wetlands or
the property of the Halifax Memorial Hospital in Roanoke Rapids, N.C.
for their construction of a new Medical Office Building. You will
recall that this area is located above the Headwater of an unnamed
tributary to Chockoyotte Creek. Refer also to our site visits and
subsequent correspondence regarding this project.
By my letter dated January 5, 1993, a mitigation plan was submitted
as requested by NCDEM and consisting of a series of small check dams in
the run of an existing stream. Then in late January 1.993, I notified
you by telephone that the permit issue was to be "put on hold" because
the engineering planning for the project had found the presence of
unstable soils that could require the proposed building site to be
relocated. Those investigations have continued and a decision has now
been made to relocate the site eastward and out of the jurisdictional
wetlands previously mapped on the property. A map showing the revised
site is enclosed.
Access to the new site is proposed by way of a 30-foot wide road
east-west across the northern edge of the property as shown. The
wetlands affected will be 30 feet wide by 370 feet long, or 11,100
square feet, or .25 acres. No other wetland disturbance is proposed for
this project. Accordingly, we will proceed under the prior approval of
the Corps Nationwide Permit No. 26 for projects located above the
Headwaters affecting less than one acre of wetlands, and under the prior
approval of. NCDEM General Water Quality Certification No. 2671 for
Headwater projects affecting less than one-third acre.

Certification from the Division of Environmental Management for
your project (Halifax Hospital expansion) located at N.C. 125 and
U.S. 158 in Halifax County. We wrote to you on 30 October 1992
discussing concerns that we have regarding the design of the
project and placing it on hold until those concerns are
addressed. As of today, we have not received a response to our
earlier letter. Unless we receive a written response from you by
29 October 1993, we will consider that you have withdrawn this
application and are not interested in pursuing the project at
this time.

On October 29, 1992, Mr. Karl Shaffer of the Raleigh
Regional office met with you to discuss the proposed additions to
the hospital facilities which will result in wetlands impacts.
The primary impact will be to 1.7 acres of jurisdictional
wetlands for construction of a medical arts building.
Through discussion with you, it was realized that future
potential expansion may also occur. This expansion will also
result in the need for wetlands impacts.
The policy of this Division is to assess in total all
potential impacts that may occur to a given site. In light of the
fact that future expansion may occur, it is requested that you
modify your original request for a 401 Water Quality
Certification. I have enclosed a copy for your use. Your request
should take into consideration the entire scope of all proposed
projects. It should be accompanied by a map which shows the
approved wetlands delineation by the Corps of Engineers as well
as a map which outlines the proposed development of the tract.
This development should be designed to minimize wetlands impacts
wherever feasible. It appears that (using the existing maps) the
natural drainage, along with buffered wetlands, could remain
between the area for the medical arts building and that area
denoted as "Area of Future Development".
The site visit resulted in the determination that the
wetlands on the 1.7 acre tract were of significant value for
water storage, streambank stabilization, and pollutant removal.
Your application should state what alternative measures are
proposed to mitigate these lost values. As discussed on site, the
most common method is construction of a water detention

structure. This structure should be designed to allow the normal
pool elevation to spread out into the creek channel such that
shallow water conditions prevail and emergent pond vegetation
may thrive. This pond should otherwise be left in a natural
setting with wooded vegetative buffers surrounding it. The pond
dam and its impounded area should not greatly impact the existing
floodplain which is up to 80 feet wide along the creek; i.e. the
majority of stored water should be over the creek where it has
steep sideslopes and a narrow floodplain.
Another area of wetlands was delineated near the entrance
area to the hospital, on the west side of the property. Based on
your comments, there is no intention for any prospective
development of this area. A narrow man-made ditch which lies
along the eastern edge of the property has no significant value.
Upon receipt of these materials, the Division will resume
its processing of your application for the Water Quality
Certification. This document will be required by the Corps of
Engineers prior to a 401 permit issuance, unless the wetlands
impacts can be reduced to less that one acre.

I am representing the Halifax Memorial Hospital of Roanoke Rapids,
N.C. to address their ongoing site development as it relates to
jurisdictional wetlands that have recently been determined to be present
on their property. I have visited the site and have discussed the
wetland situation with Mr. Paul Sherwood, Senior Hospital Vice President
and with Mr. Eric Alsmeyer of the Corps Raleigh Regulatory Field Office.
The Hospital has previously developed a Master Plan for the phased
expansion of regional medical service facilities that would be
internally intergrated in a manner that would best serve the public and
result in prudent use of their available property. Implementation of
the Master Plan beyond the actual Hospital building has progressed thus
far to result in construction of a Mental Health facility, a Doctor's
Building, and the presently ongoing construction of an addition to the
Hospital building.
Earlier this year, architectural design was initiated and tenant
commitments were made for the next link in the Master Plan consisting of
a Medical Office Building. As a part of his building site planning, the
architect contacted the Corps of Engineers to determine any potential
wetland problems that may be present. The Corps examined the Medical
Office Building site and much of the remaining Hospital property and
determined that essentially all of the proposed building site is a
jurisdictional wetland and that considerable additional Hospital
property appears to be wetland as well.

Today, several steps are being taken to address the wetland
situation on the Hospital property:
1. The attached predischarge notice is being filed for the
disturbance of up to 1.7 acres of wetlands to allow the ongoing
design efforts to proceed on the Medical Office Building and
avoid the costs of delay and/or redesign. Wetlands at this
location were previously delineated by the Corps.
2. The following schedule has been set for the determination of the
remaining wetland locations throughout the Hospital property:
a. All of the wetlands throughout the property will be delineated
by October 16, 1992.
b. Surveying and the preparation of a draft wetland map will be
completed by October 23, 1992.
c. Examination of the delineation and survey will be made by the
Corps on October 27, 1992.
Once the onsite wetlands have been mapped, future implementation of
the Master Plan will be revised to avoid, to the maximum extent
practicable, additional wetland disturbance; and where future wetland
disturbance is foupermit applications will be
filed complete wi igned to offset the wetland
losses.
