I have two suggestions for your Unit and I thought I would send this to you and you can direct it to the correct
person in DWR
In February 2015 1 will need to submit the application form for making a Permittee name change to a Land
Application of Residual Solids Permit.
I looked on the NC-DWR website for the required form but failed miserably.
Thankfully Jim Barber in the Fayetteville Regional Office was able to find the "Non -Discharge Permit
Ownership/Name Change" forrn NDPONC 11-131 under the Wastewater Irrigation Systems tab.
I suggest that you relocate the link for this form to the Miscellaneous Documents tab, which is where I initially
looked.
In filling out the Form NDPONC 11 -13, 1 have discovered that none of the lines for the certification sections on
Page 2 of 2 can be edited, i.e. they are not the fill-in the blank format as is the rest of the form.
I suggest that the lines of Page 2 of 2 be converted to a fill-in format. In other words all of the entries except the
signature lines.
