Hi Stephanie-
• The representative for Leyland Twin Lakes LLC has apparently changed from Jason Hess to Howard Kaufman.
Jason Hess's name was a holdover from the previous application, which I overlooked and should have changed
to Howard Kaufman.
• Please see the attached impact maps. I am so sorry those were not included with my application. That was a big
oversight on my part.
• No, the 63 sf of proposed impacts for the sanitary sewer connection are no longer applicable. Hopefully, having
the revised impact maps will help clarify.

I am currently reviewing the above reference project and have a few questions/requests in order to finish the review:
• Can you provide an agent authorization form signed by Mr. Jason Hess
• I do not see the buffer impacts on the site plan 20181113-1 see the reference on the impact in the form, but I am
not seeing them on the map.
• 1 understand these buffer impacts replace the buffer impacts from 20180314 version 1. Is the sanitary sewer
connection authorization for 63 s.f. from version 1 still applicable? Just need clarification on how I will word the
authorization since these replace the previous impacts.
