Division of Water Resources INSTRUCTIONS FOR FORM: FTA 04-16 & SUPPORTING DOCUMENTATION
This application is for sewer extensions involving gravity sewers, pump stations and force stains, or any combination that has been
certified by a professional engineer and the applicant flint the project meets the requirements of 15A NCAC 02T and the Division's
Minimum Design Criteria and that plans, specifications and supporting documents have been prepared in accordance with, ISA
NCAC 02T, ISA NCAC 02T .0300 Division policies and good engineering practices.
While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T .0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in 15A NCAC 02T .0305 and the applicable minimum design criteria. These
documents shall be available upon request by the Division.
Projects that are deemed pennitted (do not require a permit from the Division) are explained in ISA NCAC 02T.0303

General - When submitting an application, please use the following instructions as a checklist in order to ensure all required items
are submitted. Adherence to these instructions and checking 11he provided boxes will help produce a quicker review time and
reduce the amount of requested additional information. Failure to submit all required items will necessitate additional
processing and review time, and may result in return of the application. Unless otherwise noted, the Applicant shall submit
one original and one copy of the application and supporting documentation.

provided from a person who meets the criteria in 15A NCAC 02T .0106(b).

The flow acceptance indicated in form FTSE must not expire prior to permit issuance and must be dated less than one
year prior to the application date.
Y Submittal of this application and form FTSE indicates that owner has adequate capacity and will not violate G.S. 143-
21_ 5.67{a}.
D Intergovernmental agreements or other contracts will not be accepted in lieu of a project -specific FTSE.
F. Site Maps (All Application Pacicnges):
® Submit an 8.5-inch x l 1-inch color copy of a USGS Topographic Map of sufficient scale to identify the entire project
area and closest surface waters.
D Location of the project (gravity sewer, pump stations & force main)
D Downstream connection points and permit number (if known) for the receiving sewer
® Include a street level map (aerial) showing general project area so that Division staff can easily locate it in the field.
G. Existing Permit (All Modification Packages):
❑ Submit the most recently issued existing permit.
❑ Provide a list of any items within the permit the Applicant would like the Division to address during the permit
modification (i.e., pencil description, flow allocation, treatment facility, etc.).
H. Power Reliability Plan (Required if portable reliability option utilized for Pump Station):
❑ Per 15A NCAC 02T .0305(h)(1). submit documentation of power reliability for pumping stations.
D This alternative is only available for average daily flows less than I S,000 gallons per day
D It shall be demonstrated to the Division that the portable source is owned or contracted by the applicant and is compatible
with the station. The Division will accept a letter signed by the applicant (see 15A NCAC 02T .0106(b)) or proposed
contractor, slating that "the portable power generation unit or portable, independently -powered pumping units, associated
appurtenances and personnel are available for distribution and operation of this pump station."
D If the portable power source or pump is dedicated to multiple pump stations, an evaluation of all the pump
stations' storage capacities and the rotation schedule of the portable power source or pump, including travel
tinteframes, shall be provided in the case of a multiple station power outage. (Required at time of certification)
I. Certificate of Public Convenience and Necessity (All Application Packages for Privately -Owned Public Utilities):

Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by
the sewer extension, or

application for a franchise has been received and that the service area is contiguous to an existing franchised area or that
franchise approval is expected.

III. Certification Statement:
I Chandra C. Farmer, P.E. certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the
receiving wastewater treatment facility and that the flow from this project is not anticipated to
cause any capacity related sanitary sewer overflows or overburden any downstream pump station
en route to the receiving treatment plant under normal circumstances, given the implementation of
the planned improvements identified in the planning assessment where applicable. This analysis
has been performed in accordance with local established policies and procedures using the best
available data. This certification applies to those items listed above in Sections I and II plus all
attached planning assessment addendums for which I am the responsible party. Signature of this
form indicates acceptance of this wastewater flow.

Given that:
a. The proportion and amount of Obligated, Not Yet Tributary Daily Flow (C) accounts for
100 % and 0 MOD of the Available Capacity (E) in Pump Station
Buffalo Creek ; and that
b. The rate of activation of this obligated, not yet tributary capacity is currently approximately
0.001 MOD per year; and that
c. A funded Capital Project that will provide the required planned capacity, namely
Buffalo Creek WWPS Upgrade & FM is in design or under construction with
planned completion in 2020 ' ; and/or
d. The following applies:
The Buffalo Creek WWPS is currently rated at 2980 GPM. It has a permitted capacity of
1,716,480 gpd and a firm capacity of4,291,200 gpd. The average metered flow to the
station is 2,038,465 gpd. A project is slated to be completed in 2020 that wiII increase the
capacity of the station to 5,000 gpm, 7,200,000 gpd of firm capacity and 2,880,000 gpd of
permitted capacity.
Therefore:
Given reasonably expected conditions and planning information, there is sufficient justification to
allow this flow to be permitted, without a significant likelihood of over -allocating capacity in the
system infrastructure.
I understand that this does not relieve the collection system owner &•om complying with G.S. 143-
2I5.67(a) which prohibits the introduction of any waste in excess of the capacity of the waste
disposal system.

On February 22, 2005, the Division of Water Quality received a flow reduction request for
residences in Johnston County. The letter requests an allowable flow rate of 80 GPD per residential
bedroom based on the evaluated data for three similar existing subdivisions.
The water use data provided between November 2003 and October 2004 with daily readings
taken during November 2004 was analyzed by central office and regional office staff with additional
mfc.^ -na;icn m quested. Based upon this data and the County response to our addttio� gal inforr iauon
requests, the Division hereby approves the use of 80 GPD per residential bedroom, with a minimum
of 160 gpd for one and two bedroom units in all applicable non -discharge permit applications
Flow shall be incremented by 801gpd for each bedroom above two. Regardless of the adjusted
design daily wastewater flow rate, at no time shall the wastewater flows exceed the effluent limits
defined in the NPDES permit for the treatment facility or exceed the caDacity of the sewers
downstream of any new sewer extension or service connection(s). A permit modification and fee is
required to apply this flow reduction to any existing permits where flows have been allocated.

Re: Proposed Sewer Allocation on Buffalo Road
Dear Mr. Scott:
Johnston County has received a copy of the letter from Donnie Adams, PE dated June 26, 2018
requesting sewer allocation for the Buffalo Road Tract (East River). It is the County's understanding that
the proposed project will discharge wastewater into the existing 24" gravity sewer main along Buffalo
Road (currently owned by Johnston County). The existing 24" gravity sewer main receives flow from the
Town of Smithfield and other County bulk customers. Per our prior communication, the 24" gravity
sewer main has capacity for up to 350,000 gallons per day from the Town of Smithfield.
Futhermore, the County owns and operates the downstream facilities for transmission to the
wastewater treatment plant. The County's downstream wastewater pump station (Buffalo Creek
WWPS) is currently at capacity. However, the County has a project under design to increase the Buffalo
WWPS capacity with construction expected to begin in the fall of 2018. Construction is expected to be
completed during the summer of 2019. The existing project will provide sufficient capacity for at least
the first three phases of the proposed development. Furthermore, the County has future projects
included in the approved Capital Improvements Plan to allow for additional wastewater capacity to
accommodate future flows from the Town of Smithfield.

Jason and Ted,
The project under design has been slightly delayed, due to downstream conditions changing with master
planning. However, the County has not experienced any problems with the existing wastewater pump station
(Buffalo WWPS), and the proposed flow is an insignificant amount compared to the entire station daily
flow. The station's firm capacity can handle the additional flow from Phase 1 with no problems. The station
upstream (Selma WWPS & EQ facility) of the Buffalo wastewater pump station is an equalization facility that can
be used to manage flows to the Buffalo station. We, therefore, request Phase 1(9,600 gpd) be allowed to be
permitted. However, future phases should be held until the station upgrades are completed.

Chandra, can you provide an update on the anticipated completion date of the upgrades to the Buffalo Creek
PS?
I'll need to talk with co-workers, but we may consider including a special condition in the application that the
proposed East River sewerline will not be able to be put into operation until the upgrades to Buffalo Creek PS
are complete and an Engineering Certification for that project has been received by this office, since it is
currently 430,000 gallons over design capacity.

Good afternoon and thanks for getting back to me with this.
The Town and the Consultant will be sure to include the flow reduction letter on ALL future submittals. As things
are now, the request was for 40 units at 360 GPD ]3-bedroom), totaling 14,400 GPD. If the reduction is taken
into consideration, the request would actually be 40 units @ 240 GPD, for a total of 9,600 GPD. Something
consider...
As to the timing of the subdivision: I am confident the developer will be looking to break ground at his earliest
opportunity — but I cannot speak for him to pinpoint any exact time frame. The first phase (40 units) will likely
seek to build this Spring (2019) and sell as soon as possible. To that end, we requested availability from the
County at the downstream Buffalo Creek pump station for the first phase, only. The attached letter references
the County approval for this request. According to the letter, the County is looking to upgrade this lift station,
with construction ending by summer of 2019, rather than the 2020 date you referenced. The 2019 timing should
work well with the developer, as Phase 1 will be under construction at that time.

We are reviewing the referenced sewer extension application and have the following questions:
1. Section Vi.6 was checked "yes" for flow reduction, but a copy of the flow reduction approval was not
included, as it required by this section. In the future, please make sure to include the attached flow
reduction letter in application submittals.
As shown on the flow tracking form, the downstream Buffalo Creek pump station currently doesn't have
the available capacity for this proposed project. The pump station has -0.43 MGD capacity (this should
be shown on the form, instead of a T"). The addendum states that this pump station is being upsized,
but the upsize isn't slated to be completed until 2020. Do you not plan on putting the East River Phase 1
Project into operation until improvements to the Buffalo Creek PS are complete?
