1) This renewal shall be attached to Permit No. 93-86, which was issued to the permittee on 5/15/86, as
well as all subsequent modifications, refinements, and renewals, and copies of all documents shall be
readily available on site when a Division representative inspects the project for compliance.
2) All conditions and stipulations of the active permit remain in force under this renewal.
NOTE: A permit renewal application processing fee of $100 was received by DCM for this project.
This permit action may be appealed by the permittee or
other qualified persons within twenty (20) days of the issuing
date.
This permit must be accessible on-site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval.
All work must cease when the permit expires on
December 31, 2020
In issuing this permit, the State of North Carolina agrees
that your project is consistent with the North Carolina Coastal
Management Program.

Please accept this request for renewal of our above referenced permit that expires on
December 31,2018.
Waff Contracting has had been extremely busy and has had to overcome some major setbacks
within the last year. We have driven additional piles under our concrete supported deck and we
also made improvements to our access areas. It is our intent to complete the remaining
permitted work with our resources as our work load allows.
Please find the attached check in the amount of $100 for the renewal.
