Thank you for your email dated December 17, 2018, which included a DWR request for additional information.
We believe the following response will fully address these comments:
DWR Comment #1- "Please provide more details regarding avoidance and minimization of the S2
culvert impact for this project. The provided impact map Figure 5a does not provide sufficient detail on
the proposed project purpose of providing parking and storage for a commercial business. Please
update the plan sheet to include a schematic layout of the anticipatedparking needs and storage areas
as described in your email dated November 2, 2018. Please configure the parking spaces to avoid or
minimize the impacts to stream S2. [15A NCAC 02H.0506(fi and (g)j. "
The parking lot is the appropriate size to accommodate 28 vehicles, including: 7 company vehicles,
5- full size vans, 2 full size pickups, 14 employee vehicles. The applicant also requires this lot to
provide additional storage capacity for staging building materials. Using standard -size parking space
dimensions of 8 by 16 feet, the square footage required for parking would be at minimum 3,584 sqft.
An additional 1,500 sqft is set aside for building material storage capacity. The remainder of the
10,890 sqft lot is used for maneuvering vehicles into spaces and around the lot. A conceptual layout
for parking and materials staging is included in Attachment A (Figure 7), however, the parking lot
will not be a painted with space dividing lines. The schematic provided is simply to illustrate the
approximate area required by the applicant.
DWR Comment #2- "Please update the impact table to include the temporary impacts associated with
installing the culvert "in the dry". [15ANCA C 02H.0502(b)(3)j"
Permanent impacts to UT to Gap Creek include the installation of a 100 ft long, 36" diameter culvert
as fill. Temporary impacts include 20 ft (10 ft upstream of proposed culvert and 10 ft downstream of
proposed culvert) to construct coffer dams as part of a dewatering system, for the purpose of
installing the culvert "in the dry." An updated impact table (PCN Form page 4 of 10), is included in
Attachment A.

On November 5, 2018, the Division of Water Resources (Division) received your application dated November 1, 2018,
requesting a 401 Water Quality Certification from the Division for the above referenced project. The Division has
reviewed the information and has determined the application is incomplete and cannot be processed. The application
will be placed on hold until the following items are addressed:
1. Please provide more details regarding avoidance and minimization of the S2 culvert impact for this project. The
provided impact map Figure 5a does not provide sufficient detail on the proposed project purpose of providing
parking and storage for a commercial business. Please update the plan sheet to include a schematic layout of

Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received within 30 days, the Division
will be unable to approve the application and it will be returned.
