Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The existing conditions of the stream are a highly eroded stream that sees extremely flashy flows due to upstream impervious. The stream channel has mostly vertical side slopes that go to the edge of the townhomes on one side and a floodplain area on the other. The stream bed is made mostly of sand and stone of various sides. The stream is currently eroding the bank near the townhomes at a rapid rate putting these townhomes in danger if unaltered. 4b. List the total estimated linear feet of all existing streams on the property: 750 4h. Explain the purpose of the proposed project:* The purpose of this project is to move the stream away from the existing townhomes and to rebuild a stable bank where the current erosion problem is occurring. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* The goal of this project is to move the existing stream away from the townhomes as it is beginning to undermine the back patios connected to the townhomes. The bank behind these patio's is vertical and bare and eroding at a rapid rate. We are proposing to move the stream away from this eroding bank and build a new stream in the existing floodplain. The eroded bank will be repaired and connected to the floodplain. The proposed stream has been designed using standard stream restoration techniques. A stormwater outlet enters into the existing stream downstream of the townhomes, this stormwater outlet will continue to be open and will connect to the proposed stream. The equipment used to construct this project will include a small excavator, a skid steer, and potentially a dump truck. 4j. Please upload project drawings for the proposed project. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: This stream restoration project was designed using natural stream channel design techniques. It includes an erosion control plan as well as avoids more established trees on the site. The project design also increases the amount of stream buffer area as well as fixes an existing erosion problem. 1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: The construction of this project will use low impact equipment to do all the grading and moving of materials. An erosion control plan will be followed to avoid any downstream sedimentation. Tree fencing will surround the site to prevent any unnecessary tree removal. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? r Yes r No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: This is a stream restoration/relocation project and does not propose an increase in the amount of impervious area on the project site. F. Stormwater Management and Diffuse Flow Plan (required by DWR) 1. Diffuse Flow Plan 1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules? r Yes r No What type of SCM are you providing? F- Level Spreader F- Vegetated Conveyance (lower SHWT) r Wetland Swale (higher SHWT) r Other SCM that removes minimum 30% nitrogen R Proposed project will not create concentrated stormwater flow through the buffer Diffuse Flow Documentation 2. Stormwater Management Plan 2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?* r Yes r No 2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)? r Yes r No Comments: No impervious surfaces proposed G. Supplementary Information 1. Environmental Documentation 1a. Does the project involve an expenditure of public (federal/state/local) funds or the use of public (federal/state) land?* r Yes r No 2. Violations (DWR Requirement) u 2a. Is the site in violation of DWR Water Quality Certification Rules (15A NCAC 2H .0500), Isolated Wetland Rules (15A NCAC 2H .1300), or DWR Surface Water or Wetland Standards or Riparian Buffer Rules (15A NCAC 2B.0200)?* r Yes r No 3. Cumulative Impacts (DWR Requirement) 3a. Will this project result in additional development, which could impact nearby downstream water quality?* r Yes r No 3b. If you answered "no," provide a short narrative description. This project is a stream restoration/relocation and does not propose any new impervious areas. Since no new impervious area Will be added there wil be no additional runoff and no impact to nearby streams 4. Sewage Disposal (DWR Requirement) 4a.
