4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
A large tree, fell across Irish Buffalo Creek, which trapped debris behind it. This caused a change in pattern with has severely eroded the Almond property. The tree and debris were
removed shortly after noticed but the formation of a transverse bar that was created before it's removal continues to direct flow straight into the outside bend. Land use is agriculture.
The Almond farm grows produce in all fields adjacent to the property.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past?*

4f. List the total estimated acreage of all existing wetlands on the property:
0
4g. List the total estimated linear feet of all existing streams on the property:
1,782 ft
4h. Explain the purpose of the proposed project:*
Debris has caused a tortuous bend to occur. A transverse bar has formed, directing flow into the outside bend.
There is extreme near bank shear stress and erosion.
The purpose of this project is to move the stream back to it's prior location and repair the banks.
This will be done with ,-hook and Toe -Wood installation on Irish Buffalo Creek.
A bank full bench and laying back of banks is proposed on Irish Buffalo Creek and Cold Water Creek
4E. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The repair on Irish Buffalo Creek will consist of installing a stream crossing, installing a stable pattern, profile and dimension. This will be done with Three J -Hooks and the installation of
toe wood. E)dsting stream length is 301 ft. Proposed channel will measure 269 ft for a net loss of 32 ft of stream to correct pattern adjustments due to debris blockage.
The Cold Water Creek side will consist of installing a bankfull bench, coir matting, seeding with native vegetation and planting a 25ft buffer on the Almond side of the creek. 215 linear
feet is proposed.
I anticipate two excavators with hydraulic thumbs and a bob cat will be used to construct the project

3j. Comments:
S1 is correction of pattern. Existing channel is 319 ft. Newchannel to correct torturous bend is 264 ft. Which gives a net loss of 55 ft. Therefore 55
feet of permanent impact and 264 ft of temporary impact has been listed for Irish Buffalo Creek.
E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The existing conditions of the bed and bank material were taken into consideration during the planning and design of this project. Areas where the
channel was fundamentally stable only a bankfull bench installation has been planned.
In areas where the channel has eroding banks and shape bends, a restoration has been planned to implement correct pattern, profile and
dimensions.
All areas worked on will have a 25 ft buffer on each side planted With native vegetation.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
We will only disturb what can be stabilized daily.
Upper section (installation of bankfull bench on 253 ft) should only take one to two days to construct and stabilize.
Lower section (restoration) should be completed in 7 to 10 working days. Anticipated weather will be monitored daily.
Design engineer will be onsite daily to ensure areas are stabilized at the end of each day.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
r Yes r No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
Only 32 ft of stream will be permanently impacted. That impact is to repair damage due to debris blockage.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
r Yes r No
If no, explain why:
Creek is not located in one of the NC Protected Riparian Buffers. Also, plan is for repair of stream bank and pattern/profile.
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?*
f Yes r No
2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)?
r Yes r No
Comments:
Steam bank repair. There is not built upon area
G. Supplementary Information
1. Environmental Documentation
1a. Does the project involve an expenditure of public (federal/state/local) funds or the use of public (federal/state) land?*
r Yes r' No
1b. If you answered "yes" to the above, does the project require preparation of an environmental document pursuant to the requirements of the National or State (North Carolina)
Environmental Policy Act (NEPA/SEPA)? *
r Yes O No
Comments:
Almond Farm was awarded Ag Cost Share funds for the proper installation of BMP's.
Almond Farm will contract the project then upon completion an approval by the design engineer.
75% of cost will be reimbursed to Almond farm.
2. Violations (DWR Requirement)

accurate calculations of distance or area are required.
