The following questions provide direction in determining when the DeparCment is
required to prepare environmental documents for state-funded construclion and
maintenance activities. Answer yuestions for Part�s A through C by checking either "Yes"
or "No". Complete Part D of the checldist when Minimum Criteria Rule categories #8,
#12(i), or #15 are used.

Project Description: Replace Bridge No. 174 with a 135' steel p1aCe girder bridge
sttucture with concrete deck at the existing location, near Waynesville, North Carolina.
The project will utilize stage construction methods during consh•uction.
AnticipaYed Permit or Consultation Requirements: Nationwide Permit No. 3& 401
Cer[ifcation. Written notification is required for this projecE due to aquatic &
environmental impacts

NWP 3 and written 401 CerYificaYiou will be required for proposed project.
Special ProjecY Information:
The bridge and surrounding area wcre surveyed on 3.15.2016 and 7.29.2018 for bats rare
plants, respeclively and no specimens or evidence were found. PoYentially suiYable habitat
near the bridge was intensively surveyed but on specimens were detected. Bridge to
bridge replacement.

. A trout moratorium may be applied to the permits at the request of WRC.
Big Creek is listed as a Class C; Tr., HQW stream type.
. High Quality Water best management practices will be utilized during
construction.
. Project will adhere to NCDOT best management practices. Avoidance of
erosion and sedimentation into Big Creek and downstream habitats.
. Trees will be cleared from October 15 to April 15 to avoid potential
roosting by Indiana bats.
