It may be that the affiliations (people) associated with the permit were changed when a request was submitted, but the
facility address was never updated. However, I'm not sure we ever received a change request if the company name was
never updated. Could you remind me of the permit COC number again?

I am having trouble finding the Name/Ownership Change Form that I submitted in January. I will just go ahead and
submit another one on-line. The name of the road changed many years ago. The strange thing is that all correspondence
sent us from DEMLR has the Smithfield Way address on it, but the COC shows the original name of the road.

Has the name changed since the last permit? Also, is the information consistent with what we currently have for the
Fermil Cca_nta.c sS_.uMM_a_ry? We asked each permittee to review that information before renewals earlier this year. The
COC reflects what we had in our database as of November 1, 2018.
If there was a name change of the facility or company (e.g., "Smithfield Packaged Foods Group" changed to "Smithfield
Packaged Meats Corp." at some point), then you need to submit a Name/0 ners_hi Cha_n.ge._ g.E. r� on-line so we can
update the database. Also, did the name of the road change from what it was before?
