This permit, issued on December 20, 2013 , is subject to compliance with the application (where consistent
with the nermit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action; or may cause the permit to he null and void
1) This renewal shall be attached to the original of Permit No. 174-06, which was issued on 11/7/06,
the minor modifications issued oil 8/31/07, 9/5/12 and 9/30/13, and copies of all documents shall be
readily available on site when Division personnel inspect the project for compliance.
2) All conditions and stipulations of the active permit remain in force tinder this renewal unless
specifically altered herein.

This permit action may be. appealed by the permittee or
other qualified persons within twenty (20) clays of the issuing
date. An appeal requires resolution prior to work initiation or
continuance as the case may be.
This permit shall be accessible on-site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modification not covered
hereunder requires further Division approval
All work shall cease when the permit expires oil
December 31, 2018
