Dear Mr. Morgan:
The Division of Water Quality has reviewed your submittal for a 401 Water Quality Certification dated June 1, 2009
for the aforementioned project. Review of your application revealed it lacking the necessary information required
for making an informed permit decision. The permit application was deficient in the following areas:
• Environmental Review Incomplete.
This project requires the completion of an Environmental Assessment (EA) document as required by the State
Environmental Policy Act (SEPA). Please submit the SEPA final approval letter after you have received it from
the State Clearinghouse.
• Errors and Omissions on the PCN Form.
1. The application was not signed in the appropriate box on pg 10.
2. Section F (1), Environmental Documentation on pg 9, was not filled out correctly. Please revise this
section.
Stormwater Management Plan is required. Please provide the necessary information requested on pg 8
of the PCN.
• Culvert Details not Included.
Please submit details of the culvert installation to include: location on the plan sheet, culvert dimensions, and
cross sectional and longitudinal profiles of the culvert installation.
• Vicinity Map not Discernible.
Please provide a better quality vicinity map.
Therefore, pursuant to 15A NCAC 2H .0507(a) (5), we will have to place the permit application on hold until we are
supplied the necessary information. You have 21 business days to respond in writing with the requested information
or notification to this office that the information is forthcoming. If, at the end of the 21 days, this office has not
received this information in writing, we will assume you are withdrawing your application and it will be returned.
