Dear Mr. Kichefski:
The North Caralina Departmer�t af Transportation is schecl�led to replace Bridge #Z3 on
NC I94 with a triple barireled reinforced cancrete box culvert. The existin� bridge is 34'
�ong x 23' wide with a steel plank deck an steel I-beams with vertica� concrete abutments.
The proposed triple barreied culvert has inlet sills to isolate baseflaw to the center banrel
along with interior si11s to retain natural bed material in the cenier barrel. The culvert wil�
be stage consta-ucted to maintain traffic on-site during constz-uction. Stream impacts are
tabulated belo�v.

Note that the stream will be shifted between barrels as per the pha�ing p�an sheets
but all o� �he temporary impacts a��ear to fal� within the e�velope of the
permanent impacts so we are nat double counting.
Construction of this project will require impacts to waters a�the United States in the New
River Basin. The existing bridge crosses Old Field Branch (Class: C; Trout, HQW). The

project is Iocated within a trout watershed. This project does not clrain to designated
critical habi�at for the elk toe mussel.
This project was reviewed far the presence af threatened ar�d endangered species in the
field and I have also included a quezy of the NCNHP database. Based on the project area
mapped with my request, t�ie database search indicates that there are no records of any
federally listed threatened or endange�-ed species, important natural communities, natural
areas, or conservation/mazaaged a�eas within the proposed pxoject boundary. There will
be 1 tree cut at tl�e project locatian due ta cut and fiil work. The NLEB is listed as a
protected species in this courrty bui the project does nat fall within �.25 miles of any red
HUC identified in the SL4PES spelled out in the Nationwide Permit Regional
Conditions. We do not anticipate any impact to any ather federally listed species.
The Eastern Hellbender is also known ta accur in the area. Surveys have not been
conc�ucted at this time but the stream is only 10' �ride and faixly shallow. With the very
limited size af the canstructian area, swrveying for these hellbenders will be faarly easy
should they become listed as a protected species in 2419.
The project will be constnzcted immediately adjacent to the L)owntown Lansing His#oric
D'zstrict. There are several buildings adjacent to the constructaon area. The use of a
culvert here helped us avoid the buildings and thus impacts to th� Historic District.
Cultural resource compliance wa�k has been completed. Na archaeo�ogica.l resources or
historic properties vv�ll be affected by this project (�ee attached). The Division I 1 Bridge
Program Manager wi�l coordinate wi#h the Town. of Lansing o� the final finish
appearance of the headwails and rails.
The project impacts are not significant in nature and shouId not alter the best usage for
this stxeam. All appropriate BMPs will be used during constructivn. Attached is a
Preconstniction Notification Form, plan sheets, and a�narked topography map.
By copy of this electronic ietter, it is requested that the Division of Water Resources,
401/Wetlands Unit, issue the apprapriate pernut for constructian of this praject. �t is
requested that any co�xunents related to the 401 eertification be electronically forwarded
directly to me with a copy sent to the US Army Corps af Engineers.

Coordinator, vv�th the North Carolina Wildlife Resources Commission comment directly
to you concez-�nung the 404 Nation�de Permit application with a copy sen� to me_
