N $PANG'1 PAI D As per our conversation on Friday July 2, 2009, this letter will serve to request a modification to DWQ Project # 09-0121. The 401 Water Quality Certification and Authorization Certificate per the Neuse Buffer Protections Rules was issued on April 7, 2009. The impacts approved in that 401 Certification allowed the construction of a road crossing included 115 linear feet of stream and 10,695 square feet of buffer. This request is being made to provide authorization for impacts that will result from the placement of an eight inch sewer line in an unnamed tributary to Richlands Creek. Twenty linear feet of stream and 3,759 square feet of buffer will be impacted by the placement of this municipal sewer line connector. Please find the following information you requested to modify DWQ Permit # 09-0121: 1. A check for $240.00; 2. Five copies of the plan sheet detailing the location of the stream and buffer impacts; 3. Five copies of the site with the locations of the impact and topo; 4. Five copies each of the Wake County Soil Map, USGS Quad map, and the two sheets that need to be changed in the PCN. This modification will allow the facility (when constructed) to attach to the municipal sewer service. Construction will be conducted by typical earth moving equipment and if the -h stream has flow, the excavation will be conducted while the stream is blocked off and pumped around.
