Bethany & Deborah, thank you for alerting us to this. The gentleman it was addressed to has been gone for a
few years so we should have updated the contact information previously. I have attached an updated Owner
Affiliation Change form with the proper contact information for James Harris, our facility’s District
Manager. We will also need to know the amount of past due payment to remit.

This COC was not renewed because fees are owed. We sent a letter back in September (attached). Please contact
Deborah Reese to take care of this. When fees have been paid, you or she must send me confirmation so that we can
renew the permit and update the website to allow COC retrieval.
