Attached, please find information pertaining to a requested Water Quality Certification associated with
a recently submitted Pre -Construction Notification for a Nationwide Permit 3 Verification request. This
request is for impacts associated with repair of an existing dam and control structure that was damaged
as a result of Hurricane Michael on October 11th, 2018. The project is located in Denver, North Carolina,
just south of NC -16 Business and north of St. James Church Road. A previous permit verification and
corresponding Water Quality Certification was recently issued for impacts associated with the
development of this project for the Action ID number referenced above.
Impacts consist of 0.13 acres of open water impacts for purpose of the dam and control structure repair
and 16 linear feet of stream impacts for purpose of riprap outlet protection. The riprap will be pressed
into the stream bed at the existing grade such that there is no loss of function. As such, compensatory
mitigation is not proposed for these additional impacts.

Impact S-5 consists of riprap stabilization and outlet protection for new impoundment and control structure. Riprap stabilization is a permanent impact
but will be pressed into stream bed at existing elevation such that aquatic life passage is preserved and there is no loss of stream function. As such,
compensatory mitigation for this impact is not proposed.

CONTRACTOR SHOP DRAWING SUBMITTALS SHALL INCLUDE BUT NOT BE LIMITED TO THE FOLLOWING ITEMS:
1. THE 60 -INCH DIAMETER ALUMINIZED CORRUGATED METAL PIPE RISER.
2. RISER STEPS.
3. BOTTOM DRAIN 8" GATE VALVE.
4. THE 8 -INCH DIAMETER RESERVOIR DRAIN VALVE, VALVE STEM AND STEM GUIDES.
5. BOTTOM DRAIN AND RISER TRASH RACKS.
6. THE 48 -INCH DIAMETER ALUMINIZED CMP BARREL AND THE MATERIAL USED TO SEAL THE JOINTS.
7. THE REINFORCEMENT STEEL FOR THE RISER FOOTING, AND CONCRETE EMERGENCY SPILLWAY.
8. CONCRETE MIX DESIGN REGARDING THE SPILLWAY RISER ANTI -FLOTATION BLOCK AND EMERGENCY SPILLWAY.
9. THE CONTRACTOR SHALL SUBMIT SHOP DRAWINGS TO STEWART, 101 N. TRYON STREET, CHARLOTTE NORTH
CAROLINA, 28202, TELEPHONE NUMBER 704-909-3516, THESE DRAWINGS ARE REQUIRED TO BE REVIEWED AND
APPROVED BY STEWART PRIOR TO ORDERING, PURCHASE OR FABRICATION OF THE RESPECTIVE ITEMS.
STEWART WILL REVIEW THE SUBMITTED MATERIAL AND RESPOND TO THE CONTRACTOR WITHIN TWO WORKING
DAYS OF RECEIVING SUBMITTAL.
CAST IN PLACE CONCRETE SPECIFICATIONS
CONCRETE FOR THE SPILLWAY RISER BASE SHALL MEET THE FOLLOWING MINIMUM SPECIFICATIONS:
1. THE MAXIMUM ALLOWABLE SLUMP SHALL BE 4 INCHES NO WATER SHALL BE ADDED TO THE MIXTURE AT THE SITE.
2. THE MINIMUM COMPRESSIVE STRENGTH SHALL BE 3,000 PSI AT 28 DAYS.
3. THE CONCRETE SHALL HAVE FROM 4 TO 6 PERCENT ENTRAINED AIR BY VOLUME
4. THE TOTAL CHLORIDE ION CONTENT OF THE CONCRETE FROM ALL SOURCES SHALL NOT EXCEED 0.08 PERCENT.
STEEL REINFORCEMENT SPECIFICATIONS
THESE SPECIFICATIONS REGARD THE STEEL REINFORCEMENT IN THE FOUNDATION FOR THE RISER FOUNDATION.
1. ALL STEEL SHALL BE GRADE 60 MINIMUM (60 KSI YIELD STRENGTH)
2. ALL REINFORCEMENT BARS SHALL BE No. 4.
3. STEEL SHALL BE PLACED AS INDICATED ON THE DRAWINGS.
4. ALL STEEL REINFORCEMENT SHALL BE FREE FROM LOOSE OR FLAKY RUST, SOIL, OIL, GREASE, PAINT OR OTHER DELETERI(
5. STEEL REINFORCEMENT AT THE SITE SHALL BE STORED ABOVE GROUND ON PLATFORMS, SKIDS OR OTHER SUPPORTS ANC
AND PROTECTED FROM MECHANICAL INJURY OR CORROSION.
6. ALL STEEL SHALL BE SHAPED BY THE MANUFACTURER IN ACCORDANCE WITH THE PROJECT REINFORCEMENT BAR SCHEDU
