Project Information and Prior Project History 3. Project Description 3a . Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The land use is residential and retail. Urban and Highly Developed 3b. List the total estimated acreage of all existing wetlands on the property: NONE 3c. List the total estimated linear feet of all existing streams (intermittent and perennial on the property: yl4' The existing stream length of Morris Branch from proposed R/W to proposed R/W is (perenni /Corp)The existing stream length of the Unnamed tributary to Morris Branch from proposed RNV to proposed R/W is +/,26<ft. (intermiltent/DWQ) 3d. Explain the purpose of the proposed project: Widening this portion of Green Level to Durham Road is necessary in order to ease projected traffic congestion in this area due increased retail and residential development. The existing two-lane facility connects two existing four lane facilities. The over capacity of the two-lane facility creates congestion that inherently creates hazards to the traveling public. 3e. Describe the overall project in detail, including the type of equipment to be used: ST-1085 is the widening of SR 1625, greenways, grading, paving, drainage structures, 2@9'x8' RCBC (Morris Branch), 1@66' RCP (Unnamed tributary) and 1@12'x10' (Greenway/Pedestrian Crossing). Major grading and paving equipment will be used. 4. Jurisdictional Determinations 4a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project. The project has been realigned to avoid filling of the unnamed tributary to Morris Branch. This realignment avoids +/-180 ft of channel. The box culvert crossing and the 66" RCP is proposed to be buried one foot at the inlet and outlet inverts. 2:1 side slopes on road fill at the site are being used to reduce the impact to streams. All stormwater will be discharged into energy dissipators or Pre-formed Scour Holes before entering the channel. Additionally the 2@9x8 box culvert will incorporate sills on the inlet and outlet of the culvert. The sill will assist in maintaining a low flow channel, which is the same width as the existing channel, through one barrel. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. Construction and Erosion and Sedimentation Control will adhere to Best Management practices to reduce sediment laden runoff. The Erosion Control plan will be reviewed by NCDEHR Land Quality. During construction of the Box culvert, the base flow will be maintained during construction. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? X Yes ? No 2b. If yes, mitigation is required by (check all that apply): X DWQ X Corps 2c.
