This type of inspection consists of two basic parts: a file review and an on-site inspection of the treatment facility.The attached inspection form notes the areas that were evaluated for this inspection.Notable findings are outlined as follows. 1. The system was turned off in April 2017 and has not been operated since.Pyramid indicated that they may have to restart the system,so they wish to keep the permit.The system should be maintained in a workable condition. 2. Please notify this office before resuming discharge. 3. The 2018 Permit Annual Fee is overdue. A copy of this invoice is enclosed.
