Dear Mr. Teal:
On November 8, 2018, Jenny Graznak and Paul DiMatteo of this office conducted a compliance
evaluation inspection on the wastewater discharge system located at the above address. The
State requires the Division of Water Resources to inspect these types of systems at least once
every five years.According to county records, you are the current owner of this property, and you
were present during the inspection.
The inspectors determined that your wastewater discharge system is not functioning properly.
The wastewater is not flowing into the septic tank from the house. Instead, the raw wastewater is
ponding in your backyard, which creates a public health hazard. You are required to fix this issue
as soon as possible. A connection to the City of Trinity sewer system is available for your home
and should be the goal if you continue to have issues with your wastewater system. You stated
that you did not currently have the funds to connect to city sewer, but that you hoped you could
soon.
Please be aware that violations of your NPDES permit, or the NC statutes and regulations under
which it is promulgated, are subject to fines of up to $25,000 per day, per violation, as set forth
in NC General Statute (NCGS) 143-215.6A, Enforcement Procedures, Civil Penalties. Your

Notes from inspection.
-The chlorinator tubes were overgrown with weeds and looked as if they had not been touched in a
while.
-The 2018 permit fee of$60 is overdue. Invoice is attached to the letter.
-After the inspection,we talked to City of Trinity collection system personnel They stated that Mr.Teal
has paid for a tap to city sewer, but that installing the private line required for connection could be
expensive due to rocks and distance. No funds are available for public assistance from City of Trinity.

Annual Permit Fee Overdue
This annual fee is required by the North Carolina Administrative Code. It covers the administrative costs associated with
your permit It is required of any person holding a permit for any time during the annual fee period, regardless of the facility's
operating status. Failure to pay the fee by the due date will subject the permit to revocation Operating without a valid
permit is a violation and is subject to a$10,000 per day fine If the permit is revoked and you later decide a permit is
needed,you must reapply,with the understanding the permit request may be denied due to changes in environmental,
regulatory, or modeling conditions.
