This email will suffice. The impacts are temporary and minimal, no mitigation is required and you provided notification when
the issue was encountered. We authorize this activity and certify that it will not result in a violation of applicable Water Quality
Standards. However, DOT must conduct the additional temporary work as described in your email dated October 4, 2018; and,
in compliance with the existing conditions of WQC004123 issued to DOT dated July 26, 2017.

Subject: U-2579C additional temporary impacts
We had an oversight between our utility plans and our permit plans on the U-2579C project. We have to open cut a small
jurisdictional channel for a 16" water line. Its shown on the attached plan sheet in green near permit site 6. We will need to
place temporary fill up and downstream of the crossing to install a pump around while the line is being laid. This will result in
25' of temporary stream impact. The material from the channel will be stockpiled and placed back in the channel. The banks
will be laid at 2:1 or flatter, lined with coir fiber matting, and revegetated with native riparian seed mix. Will this email suffice
for the additional impacts? If not I can submit a cover letter along with the attached plan sheet.

This project was permitted with 2 separate IPs. One IP is for only U-2579C and the other is part of the phased IP. The permit
that this one refers to can be found at the link below. James Lastinger did the permit for this project. Bryan has visited it in the
field for compliance and he was present (along with Monte) at the preconstruction meeting.
