Bear Lake Reserve

This letter is in response to your 10-24-18 letter to Bear Lake Reserve attached.

We did exceed daily permit limits during our second monthly sampling event on 6-21-18
with BOD of 22 mg/L, TSS 14 mg/L and NH3N of 6.1 mg/L with limits 15 mg/L BOD,
10 mg/L TSS, and 6 mg/L NH3N. The operator noted aerator failure causing these
spikes. The plant was repaired and resampled for BOD the following week with <2 mg/L
and <2 mg/L results. We did not get the NH3N and TSS results for the third week of
month until the first week of July and did not resample for these.

We will send an amended NDMR out this week with the missing Turbidity result for
6/30/18 missing from the report.

No water was irrigated or sent to irrigation holding tank in June 2018. All effluent was
recycled to the upset tank and processed again.
