Please find attached the following FTA 04-16 application package which includes all items
named on its first three pages essentially being the application, the flow tracking form, some
location maps and historical permitting in effort of extending gravity sewer main to provide
service to five lots at the terminus of Carolina Water Service's Belvedere Plantation collection
system in this location. This fifty-eight foot main extension will serve two existing recorded
residential lots at the extremity of the recently certified main permitted as WQ0039638. The
system meets Carolina Water Service specification and will be conveyed to them once certified.
