Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: All impacts to the on-site stream are avoided. A dissipator pad originally proposed to outlet within the stream channel was able to be slightly moved outside of the stream channel and now outlets to a non -stream drainageway adjacent to the stream. The width of buffer impact will be no greater than the 30' wide sewer easement. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: Construction techniques will not be able to avoid or minimize impacts to the wetland. The small wetland Will be impacted in its entirety. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? f Yes r No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: Impacts to WoUS are 0.022 acre, which is below the threshold for mitigation. F. Does the project involve an expenditure of public (federal/state/local) funds or the use of public (federal/state) land?* r Yes r No 2. Violations (DWR Requirement) 2a. Is the site in violation of DWR Water Quality Certification Rules (15A NCAC 2H .0500), Isolated Wetland Rules (15A NCAC 2H .1300), or DWR Surface Water or Wetland Standards or Riparian Buffer Rules (15A NCAC 2B.0200)?* r Yes r No 3. Cumulative Impacts (DWR Requirement) 3a. Will this project result in additional development, which could impact nearby downstream water quality?* r Yes r No 3b. If you answered "no," provide a short narrative description. The project will control runoff and water quality through on-site detention. No future impacts in the vicinity are anticipated. The parcel is expected to be developed in its entirety; no other undeveloped areas are available within the drainage area for the stream, and no water quality impacts are expected in the future. 4. Sewage Disposal (DWR Requirement) 4a. Is sewage disposal required by DWR for this project?* r Yes r Nor N/A 5. Endangered Species and Designated Critical Habitat (Corps Requirement) 5a. Is another Federal agency involved?* r Yes r No r Unknown 5e. Is this a DOT project located within Division's 1-8? r Yes r No 5f. Will you cut any trees in order to conduct the work in waters of the U.S.? r Yes r No 5g. Does this project involve bridge maintenance or removal? r- Yes r No 5h. Does this project involve (1) blasting, and/or (2) other percussive activities that will be conducted by machines, such as jackhammers, mechanized pile drivers, etc.? r Yes r No 5j. What data sources did you use to determine whether your site would impact Endangered Species or Designated Critical Habitat? USFWS records of federally protected species documented from Wake County NC were consulted, most recently on Oct. 23, 2018. NC Natural Heritage Program records dated July 2018 show no records of any naturally occurring federally protected species within 1.0 mile of the site parcel. A reintroduction site for Michaux's sumac is located within 1.0 mile of the site parcel. On-site evaluation in June 2018 by Ecological Engineering staff revealed limited habitat for Michaux's sumac, with no specimens observed. No habitat is present for red -cockaded woodpecker or mussel species. No bald eagles or eagle nests were observed on the site parcel during on-site investigation in June 2018. Consultation Documentation Upload 6. Essential Fish Habitat (Corps Requirement) 6a. Will this project occur in or near an area designated as an Essential Fish Habitat?* r Yes r No 6b. What data sources did you use to determine whether your site would impact an Essential Fish Habitat?* NOAA fisheries EFH mapper. 7. Historic or Prehistoric Cultural Resources (Corps Requirement) 7a. Will this project occur in or near an area that the state, federal or tribal governments have designated as having historic or cultural preservation status?* r- Yes r No 7b. What data sources did you use to determine whether your site would impact historic or archeological resources?* No potentially historic structures currently exist within the site parcel. The site soil has been disturbed in the past. NC State Historic Preservation Office web service shows that surveyed structures (WA -2509, Harrison Family Houses) located adjacent to the site are now gone, likely as a result of the construction of Macon Pond Road. These structures were not given any historic designation. 7c. Historic or Prehistoric Information Upload 8. Flood Zone Designation (Corps Requirement) 8a. Will this project occur in a FEMA -designated 100 -year floodplain?* r Yes r No 8c. What source(s) did you use to make the floodplain determination?* NC Floodplain Mapping Program (NCfloodmaps.com).
